摘要
目的分析行根管减压、封闭和药物治疗急性牙髓炎的效果,为临床治疗方法的选择提供参考。方法急性牙髓炎患者273例,根据治疗方法分为根管治疗组(根管减压+封闭治疗)、药物治疗组(口服清胃散颗粒治疗)和联合治疗组(根管减压+封闭治疗+口服清胃散颗粒治疗),每组91例;观察治疗后2 h 3组患者疼痛改善情况及止痛起效时间,比较3组近、远期治疗效果。结果治疗2 h后,联合治疗组疼痛缓解率(92.3%)高于根管治疗组(72.5%)和药物治疗组(58.2%)(P<0.05),根管治疗组高于药物治疗组(P<0.05);联合治疗组止痛起效时间[(34.0±12.9)min]短于根管治疗组[(51.9±14.9)min]和药物治疗组[(72.9±16.8)min](P<0.05),根管治疗组短于药物治疗组(P<0.05);联合治疗组近期、远期治疗有效率(96.7%、97.8%)高于根管治疗组(92.3%、90.1%)和药物治疗组(75.8%、63.7%)(P<0.05),根管治疗组高于药物治疗组(P<0.05)。结论根管减压+封闭联合药物治疗可有效缓解急性牙髓炎患者的疼痛,近、远期疗效确切。
Objective To analyze the efficacy of root canal decompression,sealing and drug therapy on patients with acute pulpitis to provide reference for clinical practice.Methods According to the different methods of treatment,273 patients with acute pulpitis were divided into 91 patients receiving root canal decompression and sealing(root canal therapy group),91 patients receiving oral administration of Qingweisan Granule(drug therapy group),and 91 patients receiving root canal decompression and sealing plus oral administration of Qingweisan Granule(combination therapy group).The pain relieving and the onset time of analgesics were observed.The short-and long-term efficacies were compared among three groups.Results After 2 h of treatment,the pain relieving rate was higher in combination therapy group(92.3%)than that in root canal therapy group(72.5%)and drug therapy group(58.2%)(P<0.05),and higher in root canal therapy group than that in drug therapy group(P<0.05).Analgesics came into effect earlier in combination therapy group((34±12.9)min)than in root canal therapy group((51.9±14.9)min)and drug therapy group((72.9±16.8)min)(P<0.05),and earlier in root canal therapy group than in drug therapy group(P<0.05).The short-and long-term effective rates were higher in combination therapy group(96.7%,97.8%)than those in root canal therapy group(92.3%,90.1%)and drug therapy group(75.8%,63.7%)(P<0.05),and higher in root canal therapy group than those in drug therapy group(P<0.05).Conclusion Root canal decompression and sealing combined with drug therapy can effectively relieve pain in patients with acute pulpitis,with good short-and long-term results.
