摘要
目的:探讨多层螺旋CT直接法下肢静脉成像在髂静脉压迫综合征的临床应用价值。方法:选取60例髂静脉压迫综合征患者,按成像方法不同将其分为直接法组和间接法组,每组30例。将两种方法扫描所得图像传输至后处理工作站进行三维后处理行容积再现(VR)、最大密度投影(MIP)及曲面重建(CPR)等后处理,观察和比较两组扫描时的对比剂使用量、辐射剂量和三维后处理图像质量。结果:直接法组患者的辐射剂量为(7.1±4)mGy,间接法组患者的辐射剂量为(7.3±2)mGy,两组患者的辐射剂量比较差异无统计学意义(t=-2.237,P>0.05),但直接法组所用对比剂量明显少于间接法组。将两组获取的原始图像均进行三维重建后处理,直接法组重建图像质量优于间接法组,差异有统计学意义(x^(2)=5.118,P<0.05)。结论:直接法下肢静脉CT成像方法能够清晰直观地显示髂静脉受压的部位、静脉曲张的程度及侧支循环开放的情况,且对比剂用量少。
Objective:To explore the clinical application value of lower extremity vein imaging of direct method of multi-slice spiral computed tomography(MSCT)in iliac venous compression syndrome.Method:60 patients with iliac venous compression were selected and they were divided into direct method group and indirect method group as different imaging method(direct method and indirect method),with 30 cases in each group.The obtained images of two kinds of scan methods were transferred to post-processing workstation for three-dimensional(3D)post-processing included volume rendering(VR),maximum intensity projection(MIP),curve planar reformation(CRP).The usage amount of contrast agent,the radiation dose and the image quality with 3D post-processing of two groups in scan were observed and compared.Result:The radiation doses of direct method group and indirect method group were(7.1±4)mGy and(7.3±2)mGy,respectively.And the difference of radiation dose between two groups were no significant(t=2.237,P>0.05),while the usage amount of contrast agent of direct method group was obviously less than that of indirect method group.And the post-processing with 3D reconstruction was implemented on the obtained original images of two groups,and the image quality of reconstruction of direct method group was significantly better than that of indirect method group(x^(2)=5.118,P<0.05).Conclusion:The direct method of CT imaging of the lower extremity vein can clearly and intuitively show the compression position of iliac vein,the degree of varicosity and the openness situation of collateral circulation,and the usage amount of contrast agent of this method is lower.
