摘要 目的:探讨急性心肌梗死临床检验中联合应用血浆肌红蛋白(Myo)与缺血修饰白蛋白(IMA)的诊断价值。方法:选取2019年3月-2020年8月本院收治的65例急性心肌梗死患者为试验组,于患者胸痛急性发作后不同时点采集静脉血样测定血Myo与IMA水平,统计阳性率,观察不同病理特征患者的指标差异。另选取同期65例健康体检志愿者为对照组,测定清晨空腹血Myo与IMA水平,比较观察两组指标差异。结果:试验组胸痛发作后3、6、12 h时,血Myo与IMA水平明显高于对照组,差异有统计学意义(P<0.05)。试验组各时点Myo联合IMA检测阳性率高于Myo、IMA单一检测及心电图检查阳性率。试验组病变血管数≥3支及心功能分级在Ⅲ级及以上的患者胸痛6 h时血Myo与IMA水平明显高于病变血管数≤2支和心功能分级Ⅱ级及以下患者,差异有统计学意义(P<0.05)。结论:Myo、IMA联合检测可以为早期识别急性心肌梗死提供有价值的实验室依据,且可以为临床判断急性心肌梗死病情程度提供一定参考。 Objective:To investigate the application of plasma myoglobin combined with ischemia modified albumin detection in clinical examination of acute myocardial infarction.Method:A total of 65 patients with acute myocardial infarction admitted to our hospital from March 2019 to August 2020 were selected as the experimental group.Venous blood samples were collected at different points after the acute onset of chest pain to determine blood Myo and IMA levels,and the positive rate was counted.The index difference of different pathologies characteristic patients were observed.In addition,65 healthy volunteers in the same period were selected as the control group,and the early morning fasting blood Myo and IMA levels were measured to compare and observe the difference in indicators between the two groups.Result:The blood Myo and IMA levels in the test group were significantly higher than those in the control group at 3,6,12 h after the onset of chest pain,and the differences were statistically significant (P<0.05).The positive rate of Myo combined with IMA test were higher than those of Myo,IMA single test and ECG test at each time point in the test group.The blood Myo and IMA levels of patients with diseased blood vessels≥3 and cardiac function classification grade Ⅲ and above in the experimental group were significantly higher than those with diseased blood vessels ≤2 and cardiac function classification grade Ⅱ and below at 6 hours (P<0.05).Conclusion:The combined detection of Myo and IMA can provide valuable laboratory evidence for the early identification of acute myocardial infarction,and can provide a certain reference for clinical judgment of the severity of acute myocardial infarction.
