摘要
目的:分析不同用氧浓度及用氧时间对早产儿视网膜病变的影响。方法:回顾性分析本院2020年1-6月收治的需要吸氧治疗的早产儿450例,根据吸氧浓度不同分为三组,吸氧浓度≤29%为A组,吸氧浓度30%~39%为B组,吸氧浓度≥40%为C组,各150例。对比不同吸氧时间、不同血氧饱和度早产儿的视网膜病变分期。结果:450例早产儿中,共144例患有视网膜病变,占比32.00%。其中A组20例,病变率13.33%;B组44例,病变率29.33%;C组80例,病变率53.33%;三组视网膜病变率对比差异有统计学意义(χ^(2)=55.8824,P<0.05)。吸氧<10 d患儿Ⅰ期病变率高于吸氧≥10 d患儿(χ^(2)=3.5245,P=0.0241);吸氧<10 d与吸氧≥10 d患儿Ⅱ期病变率比较差异无统计学意义(χ^(2)=2.5789,P=0.7452);吸氧<10 d患儿Ⅲ~Ⅳ期病变率低于吸氧≥10 d患儿(χ^(2)=4.5213,P=0.0011)。低、高血氧饱和度患儿Ⅰ期病变率比较差异无统计学意义(χ^(2)=2.4000,P=0.1213);低血氧饱和度患儿Ⅲ~Ⅳ期病变率低于高血氧饱和度患儿(χ^(2)=13.0213,P=0.0003)。结论:低浓度、低时间的吸氧可有效降低早产儿视网膜病变的发生率,以及病情对早产儿造成的影响,值得应用与推广。
Objective:To analyze the influence of different oxygen concentration and oxygen use time on retinopathy of prematurity in preterm infants.Method:A total of 450 cases of premature infants who need oxygen therapy in our hospital from January to June 2020 were retrospectively analyzed,and they were divided into three groups according to different oxygen concentration,oxygen concentration≤29%were the group A,oxygen concentration of 30%-39%were the group B,and oxygen concentration≥40%were the group C.The retinopathy stages of premature infants with different oxygen intake time and different blood oxygen saturation were compared.Result:Among 450 preterm infants,144 cases(32.00%)suffered from retinopathy,including 20 cases in the group A,the lesion rate was 13.33%;44 cases in the group B,the lesion rate was 29.33%;80 cases in the group C,the lesion rate was 53.33%;the retinopathy rate of the three groups was significantly different(χ^(2)=55.8824,P<0.05).The stageⅠlesion rate of children with oxygen intake time<10 d was higher than of children with oxygen intake time≥10 d(χ^(2)=3.5245,P=0.0241);there was no significant difference in the stageⅡlesion rate of children with oxygen intake time<10 d and≥10 d(χ^(2)=2.5789,P=0.7452);the stageⅢ-Ⅳlesion rate of children with oxygen intake time<10 d was lower than that of children with oxygen intake time≥10 d(χ^(2)=4.5213,P=0.0011).There was no significant difference in the stageⅠlesion rate of children with low and high blood oxygen saturation(χ^(2)=2.4000,P=0.1213);the stageⅢ-Ⅳlesion rate of children with low blood oxygen saturation was lower than that of children with high blood oxygen saturation(χ^(2)=13.0213,P=0.0003).Conclusion:Low concentration and low time oxygen inhalation can effectively reduce the incidence of retinopathy of prematurity,and the impact of the disease on premature infants,which is worthy of application and promotion.
