摘要
目的:探讨肺炎严重度指数(PSI)和CURB-65评分在评估社区获得性肺炎治疗效果中的作用。方法:选取2019年6月-2020年6月本院治疗的82例社区获得性肺炎患者。所有患者均接受PSI和CURB-65评分评估,并根据临床治疗情况分为治疗有效组与治疗无效组,比较两组PSI和CURB-65评分,分析PSI和CURB-65评分判断疾病治疗效果的敏感性、特异性。结果:82例社区获得性肺炎,治疗有效组66例(80.49%);治疗无效组16例(19.51%),治疗无效组住院时间长于治疗有效组(P<0.05)。治疗有效组入院第4天PSI和CURB-65评分明显低于入院第1天,治疗无效组入院第4天评分明显高于入院第1天(P<0.05)。PSI对治疗效果的敏感性高于CURB-65评分系统,特异性低于CURB-65评分系统,差异均有统计学意义(P<0.05)。结论:对社区获得性肺炎患者进行PSI和CURB-65评分,有利于临床判断患者的病情程度与治疗效果,值得临床应用。
Objective:To explore the role of pneumonia severity index(PSI)and CURB-65 score in the evaluation of treatment effect of community-acquired pneumonia.Method:A total of 82 patients with community-acquired pneumonia treated in our hospital from June 2019 to June 2020 were selected in this study.All patients were assessed by PSI and CURB-65 score,and were divided into the effective treatment group and the ineffective treatment group according to clinical treatment.The PSI and CURB-65 score of the two groups were compared.The sensitivity and specificity of the PSI and CURB-65 score in judging the treatment effect of the disease were analyzed.Result:There were 82 cases of community-acquired pneumonia,there were 66 cases(80.49%)in the effective treatment effect group,16 cases(19.51%)in the ineffective treatment group,and the hospital stay in the ineffective treatment effect group was longer than that in the effective treatment group(P<0.05).The PSI and CURB-65 score on the fourth day of admission in the effective treatment group were significantly lower than those on the first day of admission,and the score on the fourth day of admission in the ineffective treatment group was significantly higher than that on the first day of admission(P<0.05).The sensitivity of PSI in judging the treatment effect was higher than that of CURB-65,and the specificity was lower than that of CURB-65,and the differences were statistically significant(P<0.05).Conclusion:The PSI and CURB-65 score of patients with community-acquired pneumonia are conducive to clinical judgment of the patients’condition and treatment effect,which is worthy of clinical application.
