摘要
目的探讨新型冠状病毒肺炎(NCP)胸部高分辨率CT(HRCT)的表现及动态改变。方法18例临床确诊为NCP患者在入院时及入院后行HRCT检查,进行HRCT动态观察。结果18例NCP患者初次检查时均有异常发现,发病后1周内,27.8%(5/18)发生于单肺,病变较局限,72.2%(13/18)发生于双肺;病灶83.3%(15/18)位于中外带;其中83.3%(15/18)有磨玻璃影,斑片状磨玻璃密度影为主4例,实变为主2例,多为混合病灶改变,单发右下肺胸膜下小结节1例。发病后1~2周,83.3%(15/18)病变进展,密度增高、范围扩大,其中,明显进展5例,实变增多6例,部分合并胸膜增厚。发病2周后,61.1%(11/18)吸收好转,密度降低并条索影改变,38.9%(7/18)病变继续进展。结论HRCT能够清楚显示NCP病灶的形态和范围,反映病理改变,具有一定的特征性,可用于NCP的早期诊断,动态观察病灶变化,为临床诊疗提供重要信息。
Objective To investigate the appearances and dynamic changes of high resolution computed tomography(HRCT)ofnovelcoronavirus pneumonia(NCP).Methods 18 patients with clinically diagnosed NCPwere examined with HRCT at admission and after admission.The findings on HRCTimages were analyzed.Results All 18 patients with NCP infection had abnormal findings on the chest HRCT at their first examination.In 1 week afteronset,27.8%(5/18)lesions were distributed in the single lungand 72.2%(13/18)occurred in both lungs,most of them were localized.83.3%(15/18)were located only in the middle and inferior lobe of lung.83.3%(15/18)wereground glassinfiltration and patchy opacity in 4 cases and prominent consolidations in 2 cases,most of them were mixed lesions.There was 1 case with a small subpleural nodule in the lower lung.One to two weeks after the onset,83.3%(15/18)of the lesions progressed,with increased density and expanded scope.Among them,there were 5 cases of obvious progress,6 cases of consolidation,and part of them with pleural thickening.Two weeks after the onset of the disease,61.1%(11/18)had better absorption,decreased density and changed cord shadows,and 38.9%(7/18)continued to progress.Conclusion HRCT can clearly show the shape and scope of NCP lesions,reflect pathological changes,and has certain characteristics.It can be used for early diagnosis of NCP,dynamically observe lesion changes,and provide important information for clinical diagnosis and treatment.
