摘要
目的:探究种植体周围炎龈沟液氧化应激标记物、CRP及尿脱氧吡啶啉水平变化及意义。方法:选取2018年4月~2020年7月来我院就诊的60例种植体周围炎患者作为研究组,另选择同期60例种植体周健康者作为对照组。分别检测并比较2组龈沟液氧化应激标记物超氧化物歧化酶(SOD)、血清C反应蛋白(CRP)及尿脱氧吡啶啉(DPd)水平。研究组治疗2周后,根据急性生理学与慢性健康状况评分Ⅱ(APACHEⅡ)评分将其分为预后良好与预后不良,比较SOD、CRP与DPd水平,并用ROC评估SOD、CRP与DPd对预测种植体周围炎患者预后价值。结果:研究组患者SOD、CRP、DPd水平均显著高于对照组(P<0.05)。研究组预后良好患者SOD、CRP、DPd水平均显著低于预后不良者(P<0.05)。ROC曲线分析显示,SOD、CRP、DPd预测种植体周围炎预后AUC分别为0.785、0.806、0.826,三项联合预测AUC为0.854,敏感度为93.7%,特异度为100%。结论:种植体周围炎患者的龈沟液氧化应激标记物、CRP及尿脱氧吡啶啉水平显著升高。SOD、血清CRP、DPd对种植体周围炎预后价值评估具有重要价值。
Objective:To explore the changes and significance of oxidative stress markers,CRP and urine deoxypyridinoline in peri-implant inflammation gingival crevicular fluid.Methods:From April 2018 to July 2020,60 patients with periimplantitis who came to our hospital for treatment were selected as the research group,and 60 patients with implant circumference health were selected as the control group.The levels of gingival crevicular fluid oxidative stress marker(superoxide dismutase,SOD),serum C-reactive protein(CRP)and urine deoxypyridine(DPd)were detected and compared between the two groups.After 2 weeks of treatment for peri-implantitis in the study group,they were divided into good and poor prognosis according to the Acute Physiology and Chronic Health Status ScoreⅡ(APACHEⅡ)score.The SOD,serum CRP and DPd levels were compared and evaluated by ROC in predicting the prognosis of patients with peri-implantitis.Results:The levels of SOD,serum CRP and DPd in the study group were significantly higher than those in the control group(P<0.05).The levels of SOD,serum CRP and DPd in patients with good prognosis were significantly lower than those with poor prognosis(P<0.05).ROC curve analysis showed that the AUC of SOD,serum CRP,and DPd predicting prognosis of peri-implant inflammatory gingiva were 0.785,0.806 and 0.826,respectively.The combined prediction of the three items was 0.854,the sensitivity was 93.7%,and the specificity was 100%.Conclusion:The gingival crevicular fluid oxidative stress marker,CRP and urinary deoxypyridinoline levels in patients with peri-implantitis are significantly rised.SOD,serum CRP,DPd are of great value for prognostic evaluation of peri-implantitis prognostic value in predicting patients with peri-implantitis.
