4种常用抗精神病药物浓度监测回顾性分析 被引量:1

摘要 目的通过对4种抗精神病药物进行药物浓度监测结果的回顾性分析,为临床合理用药提供参考。方法采用液相色谱仪对氯氮平、奥氮平、帕利哌酮、利培酮进行药物浓度检测,并对其数据进行回顾性分析。结果4种药物监测1次比例门诊均大于住院患者,奥氮平在控率>帕利哌酮>氯氮平和利培酮;累计3次监测后,平均在控率均能达到50%以上,其中住院患者在控率均在70%以上,门诊患者在控率均在40%以上。氯氮平在控率提升幅度最高(84.38%),其次是利培酮(78.42%)、奥氮平(39.86%)和帕利哌酮(36.00%);氯氮平和利培酮药物浓度监测的最合适频次为2次,而奥氮平和帕利哌酮住院患者药物浓度监测最合适频次为2次,门诊患者药物浓度监测的最合适频次则为3次。结论药物浓度监测能有效提高氯氮平、奥氮平、帕利哌酮、利培酮4种药物的在控率,对临床用药具有指导作用。 Objective A retrospective analysis of the concentration monitoring results of four antipsychotic drugs was conducted to provide a reference for rational drug use in clinic.Methods The concentrations of clozapine,olanzapine,paliperidone and risperidone were detected by liquid chromatography,and the data were analyzed retrospectively.Results The proportion of 1 monitoring for 4 drugs was higher in outpatients than that in inpatients.The control rate was olanzapine>paliperidone>clozapine and risperidone.After 3 times of monitoring,the average control rate was above 50%,among which the control rate of inpatients was above 70%,and the control rate of outpatients was above 40%.Clozapine had the highest increase in control rate(84.38%),followed by risperidone(78.42%),olanzapine(39.86%)and paliperidone(36.00%).The most appropriate frequency for clozapine and risperidone concentration monitoring was 2 times,the most appropriate frequency for olanzapine and paliperidone inpatients was 2 times,and the most appropriate frequency for outpatients was 3 times.Conclusion TDM can effectively improve the control rate of clozapine,olanzapine,palisperidone and risperidone,which has a guiding effect on clinical medication.
