摘要
目的了解医院多重耐药菌(MDRO)分布和耐药性特点,为MDRO感染防控和抗菌药物选择提供参考。方法采用DL-96Ⅱ细菌测定系统,对2016年1月-2020年12月临床送检标本进行细菌鉴定和药敏试验。结果全院105369例出院患者中,各临床送检细菌培养标本34243份,检出阳性菌株9640株,其中MDRO 1597株,MDRO感染发病率为1.52%,检出率为16.57%。检出阳性菌株构成以ESBLs菌株为最多,共1002例,占62.74%,部位以下呼吸道检出最多,共731株,占45.77%,科室以重症医学科所占比例最高,为15.97%。耐药监测数据显示,MDRO革兰阴性杆菌对头孢唑林、氨苄西林、头孢曲松、头孢呋辛耐药较为严重,对美罗培南、亚胺培南、多黏菌素B和米诺环素较敏感。MRSA除对复方新诺明40.24%敏感外,其他耐药较为严重,对青霉素、苯唑西林、头孢西汀耐药率达100.00%。结论加强医院感染监测,严格抗菌药物管理,落实消毒隔离感控措施是预防MDRO产生和传播的关键。
Objective To understand the distribution and drug resistance of multidrug-resistant bacteria(MDRO)in hospital,to provide reference for the prevention and control of MDRO infection and the selection of antibiotics.Methods The DL-96Ⅱbacterial detection system was used to conduct bacterial identification and drug sensitivity test on clinical samples from January 2016 to December 2020.Results Among the 105369 discharged patients,34243 were detected by hair coloring,and 9640 were positive.Among them,1597 strains were MDRO,and the incidence of MDRO infection was 1.52%,with a detection rate of 16.57%.ESBLs was the most positive strain,accounting for 62.74%(1005/1597).731 strains(45.77%)were found in the lower respiratory tract.It accounted for the highest proportion in department of critical care medicine(15.97%).Drug resistance monitoring data showed that MDRO Gˉwas more resistant to cefazolin,ampicillin,ceftriaxone and cefuroxime,and more sensitive to meropenem,imipenem,polymyxin B and minocycline.MRSA was 40.24%sensitive to sulfamethoxazole,but the resistance rate to penicillin,oxacillin and cefoxitin was 100.00%.Conclusion Strengthening nosocomial infection monitoring,strict antimicrobial drug management,and implementing disinfection and isolation measures are the key to prevent the occurrence and spread of MDRO.
