摘要
目的:分析华中科技大学同济医学院附属同济医院在推进处方审核系统中给药途径存在的问题,根据对策构建给药途径在处方审核中的精细化审核模式,促进临床合理用药。方法:收集该院处方审核系统2020年4月-9月门诊及住院所有给药途径监测数据,对红灯警示的不适宜给药途径,进行回顾性分析。根据循证证据及其等级结合该院临床实践,探索给药途径的精细化审核规则。结果:该院2020年4月-9月给药途径红灯警示问题总数为5328例,主要发生于静滴、外用、漱口、鼻饲管注入等给药途径,根据红灯警示问题结合该院实际情况迄今共维护给药途径自定义审核规则800多条。结论:基于循证证据级别结合临床实际治疗需求构建具有医院特色的审核规则库,根据药物性质、科室治疗特色综合考虑患者病情特征进行给药途径的精细化审核,同时完善医生端HIS系统与PASS系统信息对接,对于减少医院给药途径不适宜问题,规范合理用药尤为重要。
OBJECTIVE To analyze the existing problems in the route of administration in the advancement of prescription review system at our hospital and construct a refined review mode of route of administration during prescription review according to the countermeasures to promote the rational clinical use of drugs.METHODS The monitoring data were collected for all routes of administration in our hospital’s prescription review system(PASS)from April to September 2020 in outpatient and inpatient settings.And retrospective analysis was performed for inappropriate routes of administration indicated by red light.Based upon the evidence-based evidence and its level,combined with our institutional clinical practice,refined audit rules were explored for the route of administration.RESULTS The total number of red light warning problems from April to September 2020 was 5328,occurring predominantly in intravenous infusion,external use,mouth-washing,nasogastric tube injection and other administration routes.Based upon red light warning problems and actual institutional situation,more than 800 custom audit rules for administration routes were maintained.CONCLUSION The level of evidence-based evidence should be combined with actual clinical treatment needs to construct a database of hospital-specific audit rule according to the nature of drugs and departmental treatment characteristics.Comprehensively considering the characteristics of patient conditions to refined audit the route of administration improves the information level of doctor-side HIS system.PASS system is vital for reducing inappropriate routes of administration and standardizing rational drug dosing.
