摘要
目的探讨经喉罩全身麻醉下支气管镜引导针吸活检术(TBNA)的应用价值。方法将2016年1月至2019年10月于我院呼吸内镜中心检查的92例行TBNA的患者随机分为全麻组(46例,经喉罩静脉全身麻醉)和局麻组(46例,局部利多卡因麻醉)。比较两组患者的诊断阳性率、各项指标的波动范围、操作时间及不良反应、并发症、术后不良记忆发生率。结果全麻组的诊断阳性率高于局麻组(P<0.05)。全麻组HR、RR、SBP、DBP、SpO_(2)的波动差值均小于局麻组(P<0.05)。全麻组的操作时间短于局麻组(P<0.05)。全麻组的不良反应及术后不良记忆发生率均低于局麻组(P<0.05);两组的并发症发生率比较,差异无统计学意义(P>0.05)。结论经喉罩全身麻醉下TBNA的诊断阳性率高,安全性好,患者不良记忆少,值得临床推广。
Objective To explore the application value of transbronchial needle aspiration(TBNA)under general anesthesia through laryngeal mask.Methods A total of 92 patients undergoing TBNA who were examined in the respiratory endoscopy center of our hospital from January 2016 to October 2019 were randomly divided into general anesthesia group(46 cases,general anesthesia through laryngeal mask)and local anesthesia group(46 cases,local lidocaine anesthesia).The positive rate of diagnosis,the fluctuation range of various indicators,operation time,and incidences of adverse reactions,complications and postoperative bad memory were compared between the two groups.Results The positive rate of diagnosis in the general anesthesia group was higher than that of the local anesthesia group(P<0.05).The fluctuation differences of HR,RR,SBP,DBP and SpO_(2) of the general anesthesia group were less than those of the local anesthesia group(P<0.05).The operation time of the general anesthesia group was shorter than that of the local anesthesia group(P<0.05).The incidences of adverse reactions and postoperative bad memory of the general anesthesia group were lower than those of the local anesthesia group(P<0.05);there were no significant differences in the incidences of the complications between the two groups(P>0.05).Conclusion The TBNA under general anesthesia through laryngeal mask has high diagnosis positive rate,good safety and less bad memory,which is worthy of clinical promotion.
