孕中期脐动脉S/D与脐带扭转螺距值对新生儿窒息情况的预测作用 被引量:4

Predictive effect of S/D of umbilical artery and umbilical cord torsion pitch value on neonatal asphyxia in the second trimester

摘要 目的探讨孕中期脐动脉收缩末期峰值与舒张末期峰值的比值(S/D)、脐带扭转螺距值对新生儿窒息情况的预测作用。方法回顾性分析2018年1月-2019年12月于医院分娩的有新生儿窒息高危因素的174例孕产妇的临床资料,随访其妊娠结局,将出现新生儿窒息情况的22例孕产妇纳入窒息组,其余孕产妇纳入对照组。对比2组孕中期脐动脉S/D、脐带扭转螺距值;使用受试者工作特征(ROC)曲线分析孕中期脐动脉S/D、脐带扭转螺距值对新生儿窒息的预测价值。结果174例孕产妇中,有22例的妊娠结局为新生儿窒息,发生率为12.64%;窒息组孕中期脐动脉S/D水平高于对照组(P<0.001),脐带扭转螺距值低于对照组(P<0.001);受试者工作特征曲线(ROC)分析显示,孕中期脐动脉S/D、脐带扭转螺距值预测新生儿窒息的cut-off值分别为3.95、4.37,脐动脉S/D联合脐带扭转螺距值的曲线下面积(AUC)为0.954,高于单独指标评估(P<0.05)。结论孕中期脐动脉S/D、脐带扭转螺距值对新生儿窒息情况的发生具有一定的预测作用,且二者联合的预测价值更高。 Objective To explore the predictive effect of the ratio of the umbilical artery end systolic peak to the end diastolic peak value(S/D)and the umbilical cord torsion pitch on neonatal asphyxia in the second trimester.Methods The clinical data of 174 pregnant women with high risk factors of neonatal asphyxia who gave birth in our hospital from January 2018 to December 2019 were retrospectively analyzed,and their pregnancy outcomes were followed up,and 22 pregnant women with neonatal asphyxia were included in asphyxia group,the rest of pregnant women were included in the control group.Compare the umbilical artery S/D and umbilical cord torsion pitch values in the second trimester of the two groups.Receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of umbilical artery S/D and umbilical cord torsion pitch value in the prediction of neonatal asphyxia in the second trimester.Results Among the 174 pregnant women,22 cases had a pregnancy outcome of neonatal asphyxia,with an incidence rate of 12.64%.The umbilical artery S/D level in the second trimester of the asphyxia group was higher than that of the control group(P<0.05),and the umbilical cord torsion pitch value was lower than that in the control group(P<0.001).ROC analysis showed that the cut-off value of umbilical artery S/D and umbilical cord torsion pitch in the second trimester were 3.95 and 4.37,respectively,and the AUC of umbilical artery S/D combined with umbilical cord torsion pitch was 0.954,which was higher than the evaluation of individual indicators(P<0.001).Conclusion Umbilical artery S/D and umbilical cord torsion pitch in the second trimester have certain predictive effects on the occurrence of neonatal asphyxia,and the combination of the two has a higher predictive value.
