摘要
目的检测内皮素(ET-1)、细胞间黏附分子-1(ICAM-1)在慢性牙周炎患者龈沟液中的表达水平,探讨其临床意义。方法选取2018年10月至2020年7月该院收治的慢性牙周炎患者97例及牙龈炎患者98例分别作为慢性牙周炎组和牙龈炎组,选取同期牙周健康者97例作为对照组。采用酶联免疫吸附试验(ELISA)检测各受试者龈沟液中ET-1、ICAM-1表达水平,同时检测牙周指标菌斑指数(PLI)、出血指数(BI)、探诊深度(PD)、临床附着丧失(CAL)和龈沟液中各炎症因子指标,分析ET-1、ICAM-1水平与牙周指标相关性。结果与对照组比较,牙龈炎组与慢性牙周炎组患者PLI、BI、PD、CAL、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、ET-1、ICAM-1水平明显升高,白细胞介素-10(IL-10)水平降低,差异有统计学意义(P<0.05);与牙龈炎组比较,慢性牙周炎组患者PLI、BI、PD、CAL、CRP、TNF-α、IL-6、ET-1、ICAM-1水平明显升高,IL-10水平降低,差异均有统计学意义(P<0.05)。与轻度组比较,中、重度慢性牙周炎患者龈沟液中ET-1、ICAM-1水平明显升高,差异有统计学意义(P<0.05);与中度组比较,重度慢性牙周炎患者龈沟液中ET-1、ICAM-1水平明显升高,差异有统计学意义(P<0.05)。慢性牙周炎患者龈沟液中ET-1、ICAM-1水平与PLI、BI、PD、CAL呈正相关(P<0.05)。与治疗前比较,治疗后1周慢性牙周炎组患者龈沟液中PLI、BI、PD、CAL、CRP、TNF-α、IL-6、ET-1、ICAM-1水平明显降低,IL-10水平升高,差异均有统计学意义(P<0.05)。结论慢性牙周炎患者龈沟液中ET-1、ICAM-1高表达,与疾病严重程度有关,治疗后ET-1、ICAM-1水平降低,可能通过检测ET-1、ICAM-1水平判断慢性牙周炎病情发展。
Objective To detect the expression levels of endothelin-1(ET-1)and intercellular adhesion molecule-1(ICAM-1)in gingival crevicular fluid of patients with chronic periodontitis,and to explore the clinical significance.Methods A total of 97 patients with chronic periodontitis and 98 patients with gingivitis admitted to this hospital from October 2018 to July 2020 were selected as chronic periodontitis group and gingivitis group,another 97 periodontal healthy people in the same period were selected as the control group.The levels of ET-1 and ICAM-1 in gingival crevicular fluid were detected by enzyme-linked immunosorbent assay(ELISA),the periodontal indexes such as plaque index(PLI),bleeding index(BI),probing depth(PD),clinical attachment loss(CAL)and inflammatory factors in gingival crevicular fluid were detected,and the correlation between ET-1,ICAM-1 levels and periodontal indexes was analyzed.Results Compared with those in the control group,the levels of PLI,BI,PD,CAL,C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),ET-1 and ICAM-1 in gingivitis group and chronic periodontitis group were significantly higher,and the level of interleukin-10(IL-10)was significantly lower(P<0.05).Compared with those in the gingivitis group,the levels of PLI,BI,PD,CAL,CRP,TNF-α,IL-6,ET-1 and ICAM-1 in chronic periodontitis group were significantly higher,and the level of IL-10 was significantly lower(P<0.05).Compared with those in the mild group,the levels of ET-1 and ICAM-1 in gingival crevicular fluid of patients with moderate and severe chronic periodontitis were significantly higher(P<0.05).Compared with those in the moderate group,the expression levels of ET-1 and ICAM-1 in gingival crevicular fluid of patients with severe chronic periodontitis were significantly higher(P<0.05).The levels of ET-1 and ICAM-1 in gingival crevicular fluid of chronic periodontitis was positively correlated with PLI,BI,PD and CAL(P<0.05).Compared with before treatment,the levels of PLI,BI,PD,CAL,CRP,TNF-α,IL-6,ET-1 and ICAM-1 in gingival creval fluid of chronic periodontitis group were significantly decreased one week after treatment.The levels of PLI,BI,PD,CAL,CRP,TNF-α,IL-6,ET-1,ICAM-1 in gingival crevicular fluid of chronic periodontitis group 1 week after treatment were significantly lower than those before treatment,while the level of IL-10 was increased,with statistical significance(P<0.05).Conclusion The high expression of ET-1 and ICAM-1 in gingival crevicular fluid of patients with chronic periodontitis is related to the severity of the disease.After treatment,the levels of ET-1 and ICAM-1 are decreased,which may be used to determine the development of chronic periodontitis.
