摘要
目的探讨多聚(腺苷二磷酸核糖)聚合酶14(PARP14)、P65、白细胞介素-1β(IL-1β)在胰腺癌发生、发展中的作用机制及其与临床病理特点的相关性。方法选取2011年1月至2018年12月首都医科大学附属北京朝阳医院收治的手术切除的胰腺癌组织蜡块标本229份作为研究对象,采用组织芯片及免疫组织化学法检测PARP14、P65、IL-1β在胰腺癌组织及癌旁组织的表达并分析其与临床病理特点的相关性。结果PARP14、P65、IL-1β在胰腺癌组织表达水平明显高于癌旁组织,且三者呈正相关性,差异均有统计学意义(P<0.05),与其他临床指标无关。临床分期为3、4期患者PARP14强阳性表达阳性率明显高于1、2期患者,有高血压病史患者P65表达阳性率明显高于无高血压病史者,差异均有统计学意义(P<0.05)。有神经浸润(优势比=71.309,95%可信区间:1.13~4501.50)是死亡的独立危险因素。PARP14、P65不同阳性程度患者生存时间比较,差异均无统计学意义(P>0.05),IL-1β不同阳性程度患者生存时间比较,差异有统计学意义(χ2=8.766,P=0.033),中等阳性者生存时间明显长于强阳性者,分别为21.4、14.7个月。结论PARP14、P65、IL-1β参与了胰腺癌发生、发展,为胰腺癌的治疗和判断患者预后提供了一定帮助。
Objective To investigate the role of PARP14,P65 and IL-1βin the development of pancreatic cancer and its correlation with clinicopathological features.Methods A total of 229 wax block specimens of pancreatic cancer tissue that were surgically resected and treated in Beijing Chaoyang Hospital of Capital Medical University from January 2011 to December 2018 were selected as the research subjects.The expression of PARP14,P65 and IL-1βin pancreatic cancer tissues and adjacent control tissues of 229 cases of pancreatic cancer were detected by tissue chip and immunohistochemistry,and the correlation between PARP14,P65 and IL-1βwere analyzed.Results The expression levels of PARP14,P65 and IL-1βin pancreatic cancer tissues were higher than those in adjacent control tissues and the three were positively correlated with statistical significance(P<0.05),which had no correlation with other clinical indicators.The positive rate of PARP14 expression in stage 3 and 4 were higher than that in stage 1 and 2(P<0.05).The positive rate of P65 expression in patients with a history of hypertension was higher than that in patients without a history of hypertension,with statistical significance(P<0.05).Multivariate logistic regression analysis showed that nerve infiltration(OR=71.309,95%CI:1.13-4501.50)was an independent risk factor for death.The log-rank test for PARP14 and P65 showed no statistically significant difference(P>0.05),while the log-rank test for IL-1βshowed statistically significant difference(χ2=8.766,P=0.033),and the average survival time for the medium positive intensity was higher than the average survival time for the strong positive expression(21.4 months,14.7 months,respectively).Conclusion PARP14,P65 and IL-1βare involved in the development of pancreatic cancer,which may be helpful for the treatment and prognosis of pancreatic cancer.
