体检人群甲状腺结节筛查风险分层及患恶性病的危险因素分析 被引量:9

摘要 目的探讨体检人群甲状腺结节筛查风险分层,分析其罹患恶性病的危险因素。方法选取2016年2月至2020年3月间空军军医大学第一附属25 600例体检者为研究对象,最终纳入19 800例,男10 234例,女9 566例。收集所有入选者体检资料,分析甲状腺彩超检查结果,使用Logistic回归分析恶性患病危险因素。结果共检出甲状腺结节2 178例,检出率为11.1%,男性检出率为9.0%(922/10 234),女性检出率为13.1%(1 256/9 566);甲状腺4类结节(4a、4b)阳性例数58例,其余为1~3类良性结节,甲状腺癌患病率为2.7%(58/2 187)。不同年龄、性别、肥胖、碘摄入量及血糖水平组甲状腺良性结节和甲状腺癌分布情况具有显著差异(均P<0.05);患者性别、糖尿病和碘摄入量是影响甲状腺癌的危险因素(均P<0.05)。结论体检人群甲状腺结节检出率较高,性别、糖尿病和碘摄入量是影响甲状腺癌患病的独立危险因素。 Objective To investigate risk stratification of thyroid nodules and analyze the risk factors for malignant thyroid disease. Methods With the approval of medical ethics committee of our hospital,25 600 physical examinees were randomly selected at The First Affiliated Hospital of Air Force Medical University from February 2016 to March 2020,and at last a total of 19 800 individuals were eventually enrolled,including 10 234 males and 9 566 females. The physical examination data and color Doppler ultrasound results of all participants were collected,and then the detection rate of thyroid nodules was analyzed.Logistic regression analysis was used to screen the risk factors for malignant disease. Results A total of2 178 thyroid nodules were detected among 19 800 physical examinees,with an overall detection rate of11. 1%( 2 187/19 800),males accounting for 9. 0%( 922/10 234) and females accounting for 13. 1%( 1 256/9 566). Fifty-eight participants were detected to have positive type 4( type 4 a and type 4 b) thyroid nodules,the rest have type 1 to 3 benign nodules,and the prevalence of thyroid cancer was 2. 7%( 58/2 187). The distribution of benign thyroid nodules and thyroid cancer showed significant difference in participants of different age,gender,obesity,iodine intake and blood glucose levels( all P<0.05). Moreover,gender,diabetes and iodine intake were the risk factors for thyroid cancer( all P<0.05). Conclusion The detection rate of thyroid nodules is relatively high among physical examinees,and gender,diabetes mellitus and iodine intake are independent risk factors for thyroid cancer.
