摘要
目的:探讨肝硬化患者胆囊超声影像特点以及与肝脏血流动力学、肝功能和Child-Pugh分级之间的关系。方法:回顾性分析2015年9月至2019年8月在我科就诊的96例肝硬化患者及83例非肝硬化体检者的资料,观察胆囊彩色多普勒超声的不同表现;分析肝硬化病人胆囊壁厚度与门静脉宽度、血清白蛋白、Child-Pugh分级之间的关系。结果:较之非肝硬化体检者,肝硬化患者胆囊壁厚度明显增加(P<0.05),且双边征更多,胆囊结石发生率更高;肝硬化患者中,胆囊壁厚度与门静脉宽度呈正相关(r=0.8818,P<0.01),与血清白蛋白呈负相关(r=-0.729,P<0.01);Child-Pugh分级A、B、C三组病人胆囊壁厚度呈递增趋势。结论:胆囊壁超声表现在非肝硬化体检人群及不同Child-Pugh分级肝硬化患者有不同的特点,且与门静脉宽度及白蛋白有相关性;因此超声监测胆囊壁厚度对评估肝硬化患者肝脏血流动力学及肝功能的变化过程具有重要临床价值。
Objective:To explore the relationships between the features of ultrasonographic images of gallbladder and liver hemodynamics,liver function and Child-Pugh grades in patients with liver cirrhosis.Methods:The clinical data of 96 patients with cirrhosis and 83 non-cirrhotic healthy examination people in our hospital from September 2015 to August 2019 were retrospectively analyzed.The manifestations of gallbladder were observed through color Doppler ultrasound.The relationships between gallbladder wall thickness and portal vein width,serum albumin or Child-Pugh grades in patients with cirrhosis.Results:Compared with the non-cirrhotic group,cirrhotic patients showed much thicker thickness of Gallbladder wall,more Double-line Sign,higher incidence of gallstones.The thickness of gallbladder wall was positively correlated with the portal vein width(r=0.8818,P<0.01),while,negatively correlated with serum albumin concentration(r=-0.729,P<0.01)in cirrhotic patients.The thicknesses of the gallbladder wall in patients with Child-Pugh Grade A,B or C changes with upward trends.Conclusion:There were different ultrasonographic manifestations of gallbladder in Non-cirrhotic people and cirrhotic patients with different Child-Pugh Grades.There were opposite correlations between the thickness of the gallbladder wall and portal vein width or serum albumin concentrate in patients with cirrhosis.Therefore,monitoring the thickness of the gallbladder wall by Ultrasound has important clinical value in evaluating the changes of liver hemodynamics and liver function in patients with cirrhosis.
