摘要
目的探讨美容缝合技术在颌面部创伤外科整形术中的应用效果。方法回顾性分析2018年1月至2020年1月在榆林市中医医院接受颌面部创伤外科整形术的102例患者的临床资料,其中采取常规清创缝合术的49例患者为对照组,采取美容缝合术的53例患者为观察组。比较两组患者的临床指标、切口缝合效果、创口愈合情况[温哥华瘢痕评定量表(VSS)]、并发症发生率、心理状态[焦虑自评量表(SAS)、抑郁自评量表(SDS)]和满意率。结果观察组患者的手术时间为(70.11±12.30)min,明显长于对照组的(56.94±10.29)min,水肿天数、伤口愈合时间、拆线时间分别为(3.95±0.74)d、(4.68±1.03)d、(5.47±1.05)d,明显短于对照组的(7.43±1.13)d、(5.73±1.12)d、(8.61±1.48)d,差异均有统计学意义(P<0.05);观察组患者的切口缝合总有效率、满意率分别为96.23%、94.34%,明显高于对照组的83.67%、79.59%,并发症发生率为5.66%,明显低于对照组的22.45%,差异均有统计学意义(P<0.05);观察组患者的VSS评分、SAS评分、SDS评分分别为(4.71±0.95)分、(50.12±11.63)分、(49.35±10.48)分,明显低于对照组的(5.73±1.03)分、(62.40±13.42)分、(60.48±12.57)分,差异均有统计学意义(P<0.05)。结论美容缝合技术在颌面部创伤外科整形术中应用效果良好,虽然手术耗时较长,但可提高切口缝合质量,提升创口愈合速度,降低术后并发症发生率,提升美容修复效果,减少瘢痕形成造成的心理负担,提高满意度。
Objective To explore the application effect of cosmetic suture technique in plastic surgery of max-illofacial trauma.Methods The clinical data of 102 patients with maxillofacial trauma who underwent plastic surgery in Yulin Hospital of Traditional Chinese Medicine from January 2018 to January 2020 were retrospectively analyzed.Among them,49 patients who underwent conventional debridement and suture were selected as control group,and 53 patients who underwent cosmetic suture were included in observation group.The clinical indicators,incision suture ef-fect,wound healing(Vancouver Scar Scale[VSS]),incidence rates of complications,psychological status(Self-rating Anxiety Scale[SAS],Self-rating Depression Scale[SDS]),and satisfaction rate were compared between the two groups.Results The surgical time,the number of days of edema,wound healing time,suture removal time,total effec-tive rate of incision suture,satisfaction rate,incidence rate of complications,and scores of VSS,SAS and SDS were(70.11±12.30)min,(3.95±0.74)d,(4.68±1.03)d,(5.47±1.05)d,96.23%,94.34%,5.66%,(4.71±0.95)points,(50.12±11.63)points,and(49.35±10.48)points in observation group,which showed statistically significant differences with(56.94±10.29)min,(7.43±1.13)d,(5.73±1.12)d,(8.61±1.48)d,83.67%,79.59%,22.45%,(5.73±1.03)points,(62.40±13.42)points,and(60.48±12.57)points respectively in control group(P<0.05).Conclusion Cosmetic suture technique has a good application effect in plastic surgery of maxillofacial trauma.Although it has longer surgical time,it can im-prove the quality of incision suture,promote wound healing,improve the healing quality,reduce the incidence rates of complications,relieve the psychological burden caused by scar formation,and increase satisfaction.
