摘要
目的探讨^(125)Ⅰ放射性粒子近距离放射治疗口腔癌的有效性及对患者预后的影响。方法选择62例口腔癌患者,均采用^(125)Ⅰ放射性粒子近距离放射治疗。观察临床疗效、疼痛程度、生存质量、生存率及不良反应。结果62例口腔癌患者均顺利完成^(125)Ⅰ放射性粒子植入治疗,每例患者布源14~32颗,平均植入粒子19颗;颈部转移淋巴结每个布源2~5颗,平均2.8颗。原发灶治疗总有效率85.48%(53/62),其中CR、PR、SD、PD分别为29例、24例、6例、3例,转移淋巴结治疗总有效率92.00%(23/25),其中CR、PR、SD、PD分别为13个、10个、1个、1个。治疗后NRS评分为(3.29±1.13)分,低于治疗前;KPS评分为(83.61±5.17)分,高于治疗前,差异均有统计学意义(P<0.05)。62例患者均成功随访12个月,共有4例患者死亡,患者生存率为93.55%(58/62);仅有2例出现自觉轻微穿刺点疼痛,无需特殊处理。结论^(125)Ⅰ放射性粒子近距离放射治疗口腔癌可获得理想的疗效,能够缓解患者疼痛,提高生存质量,患者预后良好,且不良反应少,临床应用安全简单。
Objective To investigate the efficacy and prognosis of ^(125)Ⅰ radiation particles in patients with oral cancer.Methods 62 patients with oral cancer were selected.All the participants were treated with ^(125) Ⅰ radioactive particles.The clinical efficacy,pain degree,quality of life,survival rate and adverse reactions were observed.Results 62 patients with oral cancer successfully completed ^(125)Ⅰ radioactive particle implantation,Each patient had 14-32 buyuan,An average of 19 implanted particles;Cervical metastatic lymph nodes each source 2-5,Average 2.8;Total effective rate of treatment of primary focus was 85.48%(53/62),CR、PR、SD、PD were 29,24,6,3;Total effective rate of treatment of metastatic lymph nodes was 92.00%(23/25),CR、PR、SD、PD were 13,10,1,1;The post-treatment NRS score(3.29±1.13)was lower than the pre-treatment score,KPS score(83.61±5.17)was higher than before treatment,difference was statistically significant(P<0.05);62 patients without missing visits after treatment,All were followed up for 12 months,4 patients died,The survival rate was 93.55%(58/62);There were only 2 cases of conscious slight puncture point pain,No need for special treatment.Conclusion ^(125)Ⅰ radiation particles can be used in the treatment of oral cancer,Can relieve pain,Improve the quality of life,The patient has a good prognosis,And less adverse reactions,Clinical application is safe and simple.
