摘要
目的研究多耐药肺炎克雷伯菌的整合子基因盒及同源性分布情况。方法收集宁波市医疗中心李惠利医院多耐药肺炎克雷伯菌46株,统计分析药敏结果;采用多重PCR检测Ⅰ类、Ⅱ类、Ⅲ类整合酶基因及整合子可变区,进一步对整合子阳性菌株的可变区进行测序分析;利用多位点序列分型(MLST)对菌株进行同源性和遗传相关性研究。结果46株多耐药肺炎克雷伯菌对多种抗菌药物(氨基糖苷类阿米卡星、庆大霉素,磺胺类甲氧苄氨嘧啶)的耐药率均超过60%;Ⅰ类整合子阳性率为91.3%(40/46),未检出Ⅱ类、Ⅲ类整合子;31株菌基因盒PCR扩增阳性,其中26株dfrA12-orf1-aadA2,3株dfrA12-orf2,2株aadA2:经MLST分析,46株多耐药肺炎克雷伯菌共有7种ST型,主要流行克隆为ST11。结论本地区多耐药肺炎克雷伯菌Ⅰ类整合子携带率较高(主要含磺胺类甲氧苄氨嘧啶及氨基糖苷类耐药相关基因),分布呈一定多态性,以ST11为优势型别。
Objective To investigate the status of integrons,the relations with their carrying antibiotic resistance gene,and the status of molecular epidemiology of multi-drug resistance Klebsiella pneumonia(K.pneumoniae).Methods In total,46 strains of multi-drug resistance K.pneumonia were collected in Li Huili Hospital of Ningbo Medical Center.The classⅠ,Ⅱ,Ⅲintegrons and the variable regions of integrons were analyzed by PCR and sequencing.Seven housekeeping genes specific to K.pneumonia were amplified and sequenced.Multilocus sequence typing(MLST)was used to classify the sequence types of K.pneumonia.Results The drug resistance rates of multi-drug resistance K.pneumonia to amikacin and trimethoprim were more than 60%.40 strains carried integrinⅠ,none strains carried integronⅡand integrinⅢ.The gene most commonly found in integrons were those for aminoglycoside(aadA2)and trimethoprim(dfrA12)resistance.Multilocus sequence typing(MLST)analysis showed that ST11 was the major genotype.Conclusion In this region,multi-drug resistant Klebsiella pneumoniae had a high carrying rate of integrinⅠ(mainly containing sulfamethoprim and aminoglycoside resistance related genes),and the distribution showed a certain polymorphism,with ST11 as the dominant type.
