摘要
目的评价健脾化痰法治疗桥本甲状腺炎(HT)的临床疗效及其安全性。方法应用计算机电子检索近10年CNKI、万方、维普、PubMed等数据库中所有基于“健脾化痰”法的中药复方治疗HT的RCT研究。分别由3名研究员根据Cochrane评价手册提取出纳入文献的有效资料,交叉进行质量评价,用RevMan 5.3软件分析。结果共纳入13项研究,涉及病例844例,Meta分析结果显示:与对照组比较,基于“健脾化痰”法的中药复方治疗组临床疗效更优,有统计学意义(P<0.00001),合并检验分析结果 Z=9.41,OR=0.16,95%CI(0.11,0.24)。此外,在中医症状积分、降低TGAb及TPOAb抗体水平、减小甲状腺峡部厚度等方面治疗组更优。结论健脾化痰法治疗HT安全有效,可推广临床应用,但仍需更高质量的RCT研究支持验证。
Objective To evaluate the clinical efficacy and safety of invigorating the spleen and resolving phlegm method in the treatment of Hashimoto’s thyroiditis(HT).MethodsAll the RCT studies of HT in CNKI, Wanfang, Weipu, PubMed database based on " invigorating the spleen and resolving phlegm" were searched by computer. According to Cochrane EvaluationManual,three researchers extracted the effective data of the included research, evaluated the quality of the included literature,selected the qualified research, and then applied Revman 5.3 software for statistical analysis.ResultsA total of 13 studieswere included, involving 844 cases. The results of meta analysis showed that: compared with the control group,the traditionalChinese medicine compound treatment group based on the method of " invigorating the spleen and resolving phlegm" had betterclinical efficacy, with statistical significance(P< 0.00001). The combined test analysis results werez= 9.41,or= 0.16,95% CI(0.11,0.24). In addition, the treatment group was better in TCM syndrome score, TGAb and TPOAb antibody leveland thickness of thyroid isthmus.ConclusionThe method of invigorating the spleen and resolving phlegm is safe and effective,which can be widely used in clinical practice, but it still needs more high quality RCT research.
