摘要
目的探讨P物质(SP)在大鼠重症急性胰腺炎(SAP)肾损伤中的作用。方法48只Wistar大鼠随机分为假手术组(SO组,n=24)、重症急性胰腺炎模型组(SAP组,n=24)。SAP组用3.5%牛磺胆酸钠(1 mL/kg)按胰胆管逆行注射法制成重症急性胰腺炎模型,SO组开腹后仅轻轻翻动胰腺、十二指肠、小肠数次,随即关腹。两实验组大鼠于制模后6、12和24 h分批取胰腺及肾脏组织。光学显微镜下对胰腺以及肾脏组织进行病理学评分,全自动生化仪检测血清淀粉酶,酶联免疫吸附试验(ELISA)测定血清肌酐、SP、肿瘤坏死因子(TNF-α)、白介素-6(IL-6),应用免疫组织化学法检测肾脏中SP及其受体NK-1R的表达。结果SO组内随着时间的推移,胰腺及肾脏病理学评分、血清淀粉酶、肌酐、SP、TNF-α、IL-6、肾脏中P物质及其受体NK-1R水平的变化较小,差异无统计学意义(P>0.05)。SAP组6、12、24 h血清淀粉酶、肌酐、SP、TNF-α、IL-6、胰腺、肾脏病理评分、肾脏中P物质及其受体NK-1R的水平均较同时间点SO组明显升高,差异有统计学意义(P<0.05)。SAP组内随着时间推移,胰腺及肾脏病理学评分、血清淀粉酶、肌酐、SP、TNF-α、IL-6、肾脏中P物质及其受体NK-1R水平均较前明显升高,差异均有统计学意义(P<0.05)。结论P物质可能直接参与了大鼠重症急性胰腺炎的肾损伤过程。
Objective To investigate the role of substance P(SP)in severe acute pancreatitis(SAP)in rats with renal injury.Methods Fourty-eight wistar rats were randomly divided into shamoperated group(SO group,n=24)and severe acute pancreatitis group(SAP group,n=24).SAP model rats were established by 3.5%sodium taurocholate(1 m L/kg)injection through pancreaticobiliary duct.In SO group,abdomen was opened and then the pancreas,duodenum small intestine were turned gently several times,at last the abdomen was closed.The tissue and blood of rats were taken at 6 hours,12 hours and 24 hours after operation.The pathological changes of pancreas and kidney were observed and graded under optical microscope.The level of serum creatinine,tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and SP were determined by enzyme linked immunosorbent assay(ELISA).The level of serum amylase was measured by automatic biochemical analyzer and the expression of SP and NK-1 R of the kidney were evaluated by immunohistochemistry.Results The pathological scores of the pancreas and kidney,the level of serum amylase,creatinine,TNF-α,IL-6,SP and the expression of SP and NK-1 R of the kidney were not statistically different from those at the different time points in SO group(P>0.05).In SAP group,the pathological scores of the pancreas and kidney,the level of serum amylase,creatinine,TNF-α,IL-6,SP and the expression of SP and NK-1 R of the kidney were significantly higher than those at the same time points in SO group(P<0.05).The pathological scores of the pancreas and kidney,the level of serum amylase,creatinine,TNF-α,IL-6,SP and the expression of SP and NK-1 R of the kidney were significantly higher than those at the later time points in SAP group(P<0.05).Conclusion The SP of kidney might play a role in kidney injury in SAP of rats directly.
