摘要
目的:探究白苓参甘汤对早期胃癌(脾胃气虚证)手术患者的临床疗效及对癌胚抗原(CEA)、糖类抗原19-9(CA19-9)水平的影响。方法:回顾性分析河南中医药大学第三附属医院和郑州大学附属肿瘤医院收治的101例早期胃癌手术患者临床资料,根据治疗方法不同将患者分成对照组和观察组,对照组(50例)采用手术+化疗的治疗方案,观察组(51例)采用手术+化疗+白苓参甘汤的治疗方案,比较两组疗效、肿瘤标志物(CEA、CA19-9)水平变化、治疗前后中医症状(包括主症、次症及舌脉)积分和不良反应。结果:观察组临床有效率高于对照组(P<0.05);治疗后,两组患者中医症状积分、CEA、CA19-9均改善明显,且观察组优于对照组(P<0.05);两组患者不良反应主要为恶心呕吐、头晕头痛、肝肾功能异常、骨髓抑制、脱发等,患者均可自行缓解或经处理缓解(P>0.05)。结论:白苓参甘汤可提升早期胃癌患者术后疗效,改善患者临床症状,安全可靠。
Objective:To explore the clinical effect of Bailing Shengan Decoction and its influence to the levels of carcinoembryonic(CEA)and carbohydrate antigen 19-9(CA19-9)in patients with early gastric cancer operation(Qi deficiency of Spleen and Stomach).Methods:101 cases of early gastric cancer operation were retrospectively analyzed and divided into the control group(n=50)and the observation group(n=51)according to the treatment methods.The control group was treated with operation plus chemotherapy;on which basis,the observation group was also treated with Bailing Shengan Decoction.The curative effect,the level changes of tumor markers,the scores of TCM symptoms before and after the treatment(including primary symptoms,secondary symptoms and tongue manifestation),as well as the adverse reactions were compared between the two groups.Results:The effective rate of observation group was higher than that of the control group(P<0.05).The scores of TCM symptoms,CEA and CA19-9 were all improved after the treatment compared to those before the treatment in the two groups(P<0.05);of which the improvements in the observation group were more significant(P<0.05).The main adverse reactions were mainly nausea and vomiting,dizziness and headache,abnormal functions of liver and kidney,bone marrow suppression and hair loss in the two groups,which could be relieved with spontaneous remission or by treatments(P>0.05).Conclusion:Bailing Shengan Decoction can enhance the postoperative efficacy of early gastric cancer,it can improve the clinical symptoms,which is safe and reliable.
