摘要
目的探讨基于植物型膳食的营养支持方案对胰腺癌患者营养状态及生存质量的影响。方法从2018年6月至2020年11月于我院收治可行手术并辅以化疗胰腺癌患者中择取80例开展研究,遵随机数字表法设立对照组及观察组,对照组施以常规营养支持策略,观察组则行基于植物性膳食的营养支持方案,持续干预3个月,比较2组营养状态及生存质量。结果营养支持后,观察组血清转铁蛋白(TF)、白蛋白(ALB)及前白蛋白(PA)水平较对照组更高(P<0.05);观察组总能量高于对照组,以植物类较对照组更高(P<0.05),谷物类及动物类比较差异无统计学意义(P>0.05),观察组植物类构成比与对照组比较更高,谷物类及动物类构成比则更低(P<0.05);2组营养风险筛查量表(2002年)(NRS2002)评分均上升(P<0.05),但2组比较差异无统计学意义(P>0.05),且观察组支持后karnofsky(KPS)评分有所上升,相比于对照组更高(P<0.05);观察组生命质量测定量表(FACT-G)生理和社会家庭情况中精力不足、自感疼痛、情感状况中满意度及胰腺疾病症状中四肢乏力、油腻食物厌恶评分较对照组更高(P<0.05)。结论基于植物型膳食的营养支持方案治疗胰腺癌患者,相比于常规营养支持策略,有利于营养状态改善,且生活质量获有一定提升。
Objective To explore the effect of nutritional support program based on the plant-based diet on the nutritional status and quality of life of patients with pancreatic cancer.Methods A total of 80 patients with pancreatic cancer who were admitted to Henan Provincial People's Hospital from June 2018 to November 2020 with feasible surgery and adjuvant chemotherapy were selected for this study.These patients were divided into control group and observation group according to the random number table method.Patients in control group were given routine nutritional support strategy,whereas the ones in observation group were given nutritional support program based on plant-based diet,and the intervention was continued for 3 months.The nutritional status and quality of life for the patients in different groups were compared.Results After nutritional support intervention,the levels of serum transferrin(TF),albumin(ALB)and prealbumin(PA)in the observation group were higher than those in the control group(P<0.05).The total energy of the patients in observation group were higher than that of in control group,and the energy of plants-diet was higher than that of in control group(P<0.05),but there was no statistically significant difference for the energy between grain based diet and animal origin diet(P>0.05).The composition ratio analysis showed that the composition ratio of plants in the observation group was higher than that in the control group,while the composition ratios of cereals and meats were lower(P<0.05).The NRS2002 score increased in the two groups,but the difference between the two groups was not statistically significant,and the Karnofsky score of the observation group was increased after support intervention,which was higher than that of the control group(P<0.05).The scores of indicators in FACT-G scale,such as the anenergia,self-pain,satisfaction with emotional state,weakness in limbs,aversion to greasy food,in observation group were higher than those in control group(P<0.05).Conclusion Compared with conventional nutritional support strategy,nutritional support program based on plant-based diet for the patients with pancreatic cancer was more conducive to improving the nutritional status and enhancing the quality of life to a certain extent.
