摘要
目的分析经皮微创接骨板内固定(MIPPO)治疗对肱骨下段骨折患者血清骨钙素(BGP)、碱性磷酸酶(ALP)及关节功能评分的影响。方法选取本院2018年1月—2020年1月收治的73例肱骨下段骨折,根据治疗方法的不同分为研究组39例(采用MIPPO治疗)和对照组34例(采用切开复位普通钢板内固定治疗),比较2组围术期指标,手术前后骨代谢指标水平、关节功能,术后疗效及并发症发生情况。结果研究组切口长度、术中出血量及骨折愈合时间显著短于或少于对照组(P<0.01)。术后,2组骨钙素、碱性磷酸酶水平及Mayo评分较术前明显升高,且研究组上述骨代谢指标及Mayo评分均显著高于对照组(P<0.05,P<0.01)。研究组优良率显著高于对照组(P<0.05)。经随访,研究组未发生神经损伤,对照组仅1例发生桡神经损伤。结论采用MIPPO治疗肱骨下段骨折效果良好,可改善患者骨代谢指标水平,加快患者关节功能恢复,且损伤小、术中出血量少、安全性好。
Objective To analyze effects of minimally invasive percutaneous plate osteosynthesis(MIPPO)on serum osteocalcin(BGP),alkaline phosphatase(ALP)and joint function scores in patients with lower humeral fracture.Methods A total of 73 patients with lower humeral fracture admitted between January 2018 and January 2020 were selected and divided into research group(n=39,MIPPO treatment)and control group(n=34,open reduction and internal fixation with ordinary steel plate)according to different treatment methods.The perioperative indexes,levels of bone metabolism indexes and joint function before and after operation,postoperative efficacy and incidence rate of complications were compared between two groups.Results Values of incision length,intraoperative volume of blood loss and time of fracture healing in research group were significantly lower than those in control group(P<0.01).After operation,levels of osteocalcin,alkaline phosphatase(ALP)and Mayo score were significantly higher than those before operation in two groups,and the above levels of bone metabolism indexes and Mayo score in research group were significantly higher than those in control group(P<0.05,P<0.01).The excellent and good rate in research group was significantly higher than that in control group(P<0.05).No nerve injury was found in research group,and there was only 1 patient with radial nerve injury in control group by follow-up.Conclusion MIPPO has a good effect in treatment of patients with lower humeral fracture,which may improve levels of bone metabolism indexes and accelerate the recovery of joint function with advantages of less injury,less intraoperative volume of blood loss and good safety.
