我院从自动上报到主动监测质子泵抑制剂不良反应效果分析 被引量:3

Effect of active surveillance on adverse drug reactions induced by proton pump inhibitor compared with automatic report in our hospital

摘要 目的:探讨我院质子泵抑制剂相关血小板减少、血红蛋白减少、白细胞减少及肝肾功能损害的发生率,保障患者用药安全。方法:采用回顾性研究方法,利用“医疗机构ADE主动监测与智能评估警示系统(ADE-ASAS)”,调取我院2018年1月-2019年12月使用质子泵抑制剂的住院患者相关信息,报警病例经双人盲评确定阳性病例及药品不良反应(ADR)发生率,并评价我院ADR主动监测的质量和效果。结果:利用ADE-ASAS共纳入89570例患者,我院质子泵抑制剂致肝损害148例、白细胞减少50例、血小板减少58例、血红蛋白减少68例、中性粒细胞减少45例、急性肾损伤27例,占ADR总上报例数的21.40%,显著高于自动上报至国家不良反应监测系统的63例(4.33%)。结论:我院采用ADE-ASAS主动监测质子泵抑制剂ADR发生情况,提高了ADR筛查率和上报率,应用效果明显,有利于促进临床合理、安全用药。 Objective:To investigate the incidence of proton pump inhibitor(PPI)related thrombocytopenia,hemoglobinopenia,leukopenia and liver and kidney function damage in our hospital,in order to ensure the safety of drug use.Methods:Data of hospitalized patients using PPI in our hospital from January 2018 to December 2019 were retrospectively collected by"adverse drug events active surveillance and assessment system(ADE-ASAS)".All the alarm cases were blind evaluated by two pharmacists to determine the positive cases and the incidence of ADR induced by PPI.And the quality and effect of ADR active monitoring in our hospital were evaluated at the same time.Results:A total of 89570 patients were enrolled by ADE-ASAS,among which there were 148 cases of liver damage,50 cases of leukopenia,58 cases of thrombocytopenia,68 cases of hemoglobin decrease,45 cases of neutrophil decrease induced by PPI and 27 cases of acute kidney injury.The total incidence rate of ADR induced by PPI through ADE-ASAS was 21.40%,which was significantly higher than that of the automatical report(63 cases,4.33%).Conclusion:In our hospital,ADE-ASAS was used to actively monitor the occurrence of proton pump inhibitor related ADR,which improved the screening rate and reporting rate of ADR,and the application effect was obvious,which could contribute to clinical rational and safety of drug use.
