血清C肽与糖化血红蛋白检验诊断糖尿病的临床价值 被引量:4

摘要 目的探讨在糖尿病诊断中应用血清C肽联合糖化血红蛋白检验的价值。方法于2018年6月—2020年6月,将随机选取的96例糖尿病患者设为研究组,按糖尿病类别分为研究A组(1型糖尿病,39例)、研究B组(2型糖尿病,57例);按有无糖尿病并发症分为研究C组(无并发症,51例)、研究D组(有并发症,45例)。并将另96名正常体检者设为参比组。所有纳入研究对象均接受空腹血糖(FPG)、餐后2 h血糖(2 hPG)、血清C肽以及糖化血红蛋白(HbA1c)检验,比较不同组别检验结果。结果相较参比组,研究组患者FPG(8.76±1.49)mmol/L、2 hPG(12.48±1.52)mmol/L以及HbA1c(11.74±1.46)%水平均显著较高,差异有统计学意义(t=26.860、40.770、41.738,P<0.05);血清C肽(0.92±0.26)μg/L水平较低,差异有统计学意义(t=14.460,P<0.05)。相较研究B组,研究A组血清C肽(0.24±0.08)μg/L水平较低,差异有统计学意义(t=13.458,P<0.05);HbA1c水平比较,差异无统计学意义(χ^(2)=0.541,P=0.590)。相较研究C组,研究D组血清C肽(0.37±0.14)μg/L水平较低,差异有统计学意义(χ^(2)=12.035,P<0.05);HbA1c(13.82±1.56)%水平较高,差异有统计学意义(t=10.973,P<0.05)。结论血清C肽与糖化血红蛋白检验在糖尿病患者的临床诊断中具有较高价值,既能够联合血糖检测结果明确诊断,同时也有助于对糖尿病类型及病情进展阶段进行鉴别与判断,具有极高临床价值。 Objective To explore the value of serum C-peptide combined with glycosylated hemoglobin test in the diagnosis of diabetes.Methods From June 2018 to June 2020,96 diabetic patients randomly selected were set as the study group,and divided into study group A according to the type of diabetes(type 1 diabetes,39 cases),study group B(type 2 diabetes,57 cases);according to the presence or absence of diabetic complications,they were divided into study group C(no complications,51 cases)and study group D(complications,45 cases).And another 96 normal physical examination subjects were set as the reference group.All included subjects received fasting blood glucose(FPG),2 h postprandial blood glucose(2 hPG),serum C-peptide and glycosylated hemoglobin(HbA1c)tests,and the test results of different groups were compared.Results Compared with the reference group,patients in the study group had significantly higher FPG(8.76±1.49)mmol/L,2 hPG(12.48±1.52)mmol/L,and HbA1c(11.74±1.46)%,the difference was statistically significant(t=26.860,40.770,41.738,P<0.05);the level of serum C peptide(0.92±0.26)μg/L was lower,the difference was statistically significant(t=14.460,P<0.05).Compared with study group B,the serum C-peptide(0.24±0.08)μg/L level of study group A was lower,the difference was statistically significant(t=13.458,P<0.05);there was no statistical difference in HbA1c level(t=0.541,P=0.590).Compared with study group C,the serum C-peptide(0.37±0.14)μg/L level of study group D was lower,the difference was statistically significant(t=12.035,P<0.05);HbA1c(13.82±1.56)%level was higher,the difference was statistically significant(t=10.973,P<0.05).Conclusion Serum C-peptide and glycosylated hemoglobin tests are of high value in the clinical diagnosis of diabetic patients.They can not only be combined with blood glucose test results to confirm the diagnosis,but also help to distinguish and judge the type of diabetes and the progress of the disease.It has extremely high clinical value.
