摘要
目的探讨3D打印技术在婴幼儿狭颅症手术中的临床应用价值。方法收集婴幼儿狭颅症患者60例,分为3D打印组30例,对照组30例。3D打印组术前应用3D打印技术重建颅骨模型,辅助额眶带前移或后缩,指导狭颅症手术治疗。对照组术前采用常规影像学检查,并由同一术者完成手术。比较两组患者手术疗效。结果术后3个月随访时,3D打印组中的短头畸形头颅指数(cephalic index,CI)为(80.70±1.10),对照组为(89.30±4.50),两者比较差异具有统计学意义。3D打印组中的舟状头畸形CI为(78.30±1.40),对照组为(69.00±4.20),两者比较差异具有统计学意义。3D打印组中的斜头畸形颅顶不对称指数(anial vault asymmetry index,CVAI)为(1.60±0.80)%,对照组为(5.60±1.00)%,两者比较差异具有统计学意义。结论3D打印辅助额眶带前移或后缩技术指导狭颅症手术,能够显著提高各类婴幼儿狭颅症手术治疗效果,效果优于传统手术,值得临床推广。
Objective To explore the clinical value of 3D printing technology in infantile craniostenosis surgery.Methods A total of 60 cases with infantile craniostenosis were collected and randomly divided into 3D printing group and control group with 30 cases in each group.In 3D printing group,3D printing technology was applied to reconstruct the skull model before operation,assist the frontal orbital girdle to move forward or retracted,and guide the surgical treatment of stenosis.Routine imaging examination was performed in the control group before the operation,and the operation was completed by the same operator.Surgical efficacy of the two groups was compared.Results At the 3⁃month follow⁃up,the cephalic index(CI)of brachycephaly in the 3D printing group was(80.70±1.10)and that in the control group was(89.30±4.50).The CI of scaphoid head in the 3D printing group was(78.30±1.40)and that in the control group was(69.00±4.20).The anial vault asymmetry index(CVAI)in the 3D printing group was(1.60±0.80)%,and that in the control group was(5.60±1.00)%.All the differences were statistically significant.Conclusion 3D printing⁃assisted frontal orbital girdle advancement or retraction technology in the operation of craniotomy can significantly improve the surgical treatment of various infantile craniostenosis,and its effect is better than that of traditional surgery.It is worthy of clinical promotion.
