摘要
目的:观察2型糖尿病对急性ST段抬高型心肌梗死(STEMI)患者经皮冠状动脉介入(PCI)术后心室重构的影响。方法:选取2019年1月-2020年1月于本院确诊为STEMI并行PCI术治疗的患者80例,其中40例糖尿病患者作为DM组,40例非糖尿病患者作为non-DM组。比较两组术前及术后12、36 h的CK-MB、cTnT、NLR、hs-CPR;比较两组术后1周及末次随访的心室重构指标。比较两组不良心血管事件(MACE)发生情况。结果:术后12、36 h,DM组CK-MB、cTnT、NLR、hs-CPR均高于non-DM组(P<0.05)。DM组心室重构发生率为30.0%高于non-DM组的10.0%(P<0.05)。术后1周,两组LVEF、LVEDV、LVESV比较,差异均无统计学意义(P>0.05);末次随访时,DM组LVEF低于non-DM组,LVEDV、LVESV均高于non-DM组(P<0.05)。DM组MACE发生率高于non-DM组(P<0.05)。结论:糖尿病急性STEMI患者PCI术后发生心室重构风险更高,且MACE发生率更高。
Objective:To investigate the effect of type 2 diabetes on ventricular remodeling after percutaneous coronary intervention(PCI)in patients with acute ST-segment elevation myocardial infarction(STEMI).Method:A total of 80 patients diagnosed with STEMI and treated with PCI in our hospital from January 2019 to January 2020 were selected,among whom 40 patients with diabetes were included in the DM group and 40 patients with non-diabetes were included in the non-DM group.The CK-MB,cTnT,NLR and hs-CPR of the two groups were compared before and 12,36 h after surgery.Ventricular remodeling indexes of the two groups were compared 1 week after surgery and at the last follow-up.The incidence of adverse cardiovascular events(MACE)was compared between two groups.Result:12,36 h after surgery,CK-MB,cTnT,NLR and hs-CPR in DM group were higher than those in non-DM group(P<0.05).The incidence of ventricular remodeling in DM group was 30.0% higher than 10.0% in non-DM group(P<0.05).One week after surgery,there were no significant differences in LVEF,LVEDV and LVESV between the two groups(P>0.05).At the last follow-up,LVEF in the DM group was lower than that in the non-DM group,while LVEDV and LVESV were higher than those in the non-DM group(P<0.05).The incidence of MACE in DM group was higher than that in non-DM group(P<0.05).Conclusion:Patients with acute STEMI in diabetes have a higher risk of ventricular remodeling and a higher incidence of MACE after PCI.
