甲状腺球蛋白及其抗体联合放射性核素显像在甲状腺癌辅助诊断中的应用 被引量:5

摘要 目的探讨血清甲状腺球蛋白(TG)、抗甲状腺球蛋白抗体(TgAb)联合甲状腺放射性核素显像(ECT)在甲状腺癌患者中的诊断效果及与复发率的相关性。方法选择2017年1月至2020年1月收治的甲状腺癌患者1573例设为观察组,所有患者均经病理组织检查确诊(金标准);选择同期治疗的甲状良性疾病患者815例,设为对照1组;选择同期健康体检者938名,设为对照2组。采用全自动化学发光免疫分析仪测定各组TgAb水平;采用全自动化学发光免疫分析测定TG水平;观察组患者均采用甲状腺ECT检查,并将检查结果与与金标准进行比较;绘制受试者工作特征(ROC)曲线,分析TG、TgAb联合甲状腺ECT在甲状腺癌患者中的诊断效能;对甲状腺癌患者进行12个月随访,采用Pearson相关性分析软件对TG、TgAb水平与生存期进行相关性分析。结果观察组TG、TgAb水平均高于对照1组和对照2组(P<0.05);对照1组TG、TgAb水平高于对照2组(P<0.05);ECT检查最终确诊1434例,诊断符合率为91.16%(P>0.05);ROC曲线结果表明:TG、TgAb联合ECT在甲状腺癌患者中诊断敏感度及特异度均高于单一TG、TgAb及ECT检查(P<0.05);Pearson相关性分析结果表明甲状腺癌患者复发率与TG、TgAb水平呈正相关(r=0.801,0.738,P<0.05)。结论TG、TgAb在甲状腺癌患者中呈高表达,联合甲状腺ECT检查能获得较高的诊断效能,且与患者复发率存在强相关性。 Objective To investigate the diagnostic performance of serum thyroglobulin(TG)and anti-thyroglobulin antibody(TgAb)with or without thyroid emission computed tomography(ECT)in thyroid cancer and their correlation with the recurrence rate.Methods One thousand five hundred and seventy three patients with thyroid cancer treated between January 2017 and January 2020 were included as the study group.All patients were diagnosed by histopathology as the gold standard.A contemporary cohort of 815 patients with benign thyroid disease was selected and assigned to the control group 1;another contemporary cohort of 938 healthy checkup subjects was selected and assigned to the control group 2.Of each group,the TgAb and TG levels were determined using an automatic chemiluminescence immunoassay analyzer.The patients in the study group underwent thyroid ECT.The ECT findings were then compared with gold standard.ROC curve was generated to analyze the diagnostic efficacy of TG and TgAb with or without thyroid ECT for thyroid cancer.The follow up for patients with thyroid cancer was 12 months.Pearson correlation analysis was used to determine the correlation between TG and TgAb levels and the survival.Results The levels of TG and TgAb in the study group were higher than those in the control groups 1 and 2(P<0.05).The levels of TG and TgAb in the control group 1 were higher than those in the control group 2(P<0.05).One thousand four hundred and thirty four cases were eventually confirmed by ECT,with the coincidence rate of diagnosis being 91.16%(P>0.05).ROC curve analysis showed that the sensitivity and specificity of TG,TgAb plus ECT in diagnosing thyroid cancer were higher than those of TG,TgAb and ECT alone(P<0.05).Pearson correlation analysis showed that the recurrence rate in patients with thyroid cancer was positively correlated with the levels of TG and TgAb(r=0.801,0.738,P<0.05).Conclusion High levels of TG and TgAb are presented in patients with thyroid cancer.Levels of TG and TgAb,combined with thyroid ECT,can lead to high diagnostic efficiency,and are strongly correlated with recurrence of thyroid cancer in the patients.
