摘要
目的癌胚抗原(CEA)、糖链抗原(CA)19-9、CA724为临床上较常使用的抗原类肿瘤标志物,本研究通过检测早期结肠癌(CC)患者、结肠良性病变(CBL)患者及健康对照组的血清CEA、CA19-9及CA724水平,分析、比较这3种常规抗原类肿瘤标志物单独检测与联合检测对结肠癌早期诊断的性能。方法收集[早期(0期、Ⅰ期、Ⅱ期)CC患者157例、CBL患者130例、健康对照组170名],采用化学发光法检测研究对象血清中3种结肠癌常规抗原类肿瘤标志物CEA、CA19-9和CA724的水平,研究3种标志物检测对结肠癌早期诊断的性能。结果CEA、CA19-9和CA724对早期CC患者的水平和阳性率均高于CBL组/健康对照组(P<0.01),其中CEA的AUC为0.673,敏感度19.1%,特异度96.7%;CA19-9的AUC为0.627,敏感度12.1%,特异度97.0%;CA724的AUC为0.622,敏感度10.8%,特异度98.0%。CEA+CA19-9+CA724联合检测的AUC为0.876[95%CI(0.839,0.908)]。结论CEA、CA19-9和CA724对结肠癌的早期诊断具有一定的价值,其中以CEA对结肠癌早期患者的AUC和阳性率最高;但单个指标检测对结肠癌早期患者的阳性率不高,价值有限,联合检测可进一步提高对早期结肠癌患者的诊断性能。
Objective Carcino-embryonic antigen(CEA),carbohydrate antigen(CA)19-9 and CA724 are commonly used antigenic tumor markers in clinic.In this study,serum levels of CEA,CA19-9 and CA724 were detected in patients with early colon cancer(CC),benign colon lesions(CBL)and healthy control group(HC),to analyze and compare the performance of these three conventional antigen tumor markers in the early diagnosis of colon cancer.Methods The levels of 3 CC conventional antigen tumor markers including CEA,CA19-9 and CA724 were detected by chemiluminescence method,in the early diagnosis of CC in a large sample cohort of 157 patients with CC at early stage,144 patients with CC at late stage,130 patients with CBL,and 170 HC.The performances of conventional tumor markers for the early diagnosis of CC were evaluated.Results The levels and positive rates of CEA,CA19-9 and CA724 in patients with early CC were higher than that in CBL group/HC group(P<0.01),of which,the AUC,sensitivity and specificity of CEA,CA19-9,and CA724 were 0.673,19.1%and 96.7%,0.627,12.1%and 97.0%,and 0.622,10.8%and 98.0%,respectively.The AUC of combined detection of CEA,CA19-9,and CA724 was 0.876[95%CI(0.839,0.908)].Conclusion CEA,CA19-9 and CA724 show certain diagnostic value for the early CC,of which,CEA has the highest AUC and positive rate for patients with CC at early stage.However,each of them remains a limited diagnostic value for the early diagnosis of CC due to a dismal low positive rate in patients with CC at early stage.The combination detection contributes to the improvement of the early diagnosis of patients with CC.
