摘要
目的对主治医师的临床岗位胜任力进行评价,发现医师职业成长中的不足之处,并提出合理建议。方法遴选大内科和大外科系统临床专家以及教学、人力资源管理专家,通过专家访谈法确定主治医师临床岗位胜任力考核评价体系。应用客观结构化临床考试和临床考核方法,连续3年对主治医师开展临床岗位胜任力大赛。大赛共设置7项置信职业行为,包括指导病历书写、终末病案质控、团队抢救、医患沟通、临床查房、院感防控、不良事件处置。结果主治医师的各项能力存在不均衡性,在病历质控和临床查房方面较好,在团队抢救、医患沟通、院感防控、不良事件处置方面相对不足(P<0.001)。内、外科系统主治医师在团队抢救及医患沟通方面能力存在差异(P<0.05)。结论在主治医师的培养过程中,应加强专业技能之外的能力锻炼,以培养素质全面的临床医学人才。
Objective To evaluate the clinical competency of attending physicians,find out the deficiencies of physicians,and put forward reasonable suggestions.Methods The clinical experts and the expertise in human resource and medical teaching management were selected,and the assessment and evaluation system of clinical competence was determined through expert interview method.The objective structured clinical examination and clinical assessment methods were applied to carry out clinical competence competition for attending physicians for three consecutive years.The competition included seven confidence professional activities,including medical record writing,terminal medical record quality control,team rescue,doctor-patient communication,clinical rounds,hospital infection prevention and control,and adverse event disposal.Results The ability of attending physicians is unbalanced.They have a good command of medical record quality control and clinical ward round.They are relatively inadequate in team rescue,doctor-patient communication,hospital infection prevention and control,and adverse event disposal(P<0.001).There were significant differences in the quality control of team rescue and doctor-patient communication between the attending physicians of internal and surgical system(P<0.05).Conclusion In the process of training the attending physicians,it is suggested to strengthen the ability exercise beyond the professional skills,and cultivate clinical talents with comprehensive quality.
