肝细胞肝癌患者中MSCT三维后处理成像技术的应用 被引量:1

摘要 目的:探究多层螺旋CT(MSCT)三维后处理成像技术在评估肝细胞肝癌(HCC)患者血供方式及临床诊断中的应用价值。方法:选取2017年10月-2019年10月在本院接受治疗的HCC患者80例为对象,所有患者均接受MSCT检查和彩色多普勒超声(CDFI)检查,分析患者MSCT影像学特征,观察MSCT和CDFI下患者的血供情况,采用受试者操作特性曲线(ROC)评估MSCT对HCC患者血供方式的诊断效能。结果:80例患者手术病例检查共检出91个病灶,MSCT扫描共检出个87病灶,检出率为95.60%。DSA检查结果显示,91个病灶中,有血供83枚,无血供8枚;其中,动脉供血型68枚,非动脉供血型15枚;MSCT的血供检出率、动脉供血型血供检出率均高于CDFI(P<0.05)。ROC曲线分析结果显示,MSCT和CDFI的AUC值分别为0.750和0.657,MSCT的诊断敏感度和准确度均高于CDFI(P<0.05),两种检测方法的诊断特异度无明显差异(P>0.05)。结论:对HCC患者利用MSCT三维后处理成像技术进行检查,诊断效果好准确率高,较CDFI可提高病灶血供的检出率,对病灶血供方式的诊断具有较高的敏感度和准确度,可为HCC患者的临床诊断及治疗提供参考依据。 Objective:To explore the application value of multi-slice spiral CT(MSCT)three-dimensional post-processing imaging technique in blood supply evaluation and clinical diagnosis of patients with hepatocellular carcinoma(HCC).Methods:A total of 80 patients with HCC who were treated in the hospital from October 2017 to October 2019 were recruited as the subjects.All of them completed MSCT and color Doppler ultrasound(CDFI)examinations.The MSCT imaging characteristics of patients were analyzed,and blood supply was observed under MSCT and CDFI.The diagnostic efficiency of MSCT for blood supply in patients with HCC was evaluated with the receiver operating characteristic(ROC)curve.Results:A total of 91 lesions were detected in the 80 patients,and 87 lesions were detected by MSCT,with a detection rate of 95.60%.DSA results showed that among the 91 lesions,there were 83 with blood supply and 8 without,68 with arterial blood supply and 15 without.The detection rates of blood supply and arterial blood supply by MSCT were higher than those by CDFI(P<0.05).ROC curve analysis showed that AUC values of MSCT and CDFI were 0.750 and 0.657,respectively.The diagnostic sensitivity and accuracy of MSCT were higher than those of CDFI(P<0.05),but there was no significant difference in diagnostic specificity between the two(P>0.05).Conclusion:The application of MSCT three-dimensional post-processing imaging technique in examination of patients with HCC can achieve good effects,with high accuracy.Compared with CDFI,it can improve the detection rate of blood supply of the lesion,and the diagnostic sensitivity and accuracy for blood supply mode of the lesion are higher.It can provide reference for clinical diagnosis and treatment of HCC.
