摘要
目的调查重庆市某地区临床护士下肢静脉曲张发生现状,分析其影响因素,为进一步干预和提高防护水平提供依据。方法采用方便抽样方法,选取2019年6-12月重庆市某地区1所三级甲等综合医院、7所二级医院、15所乡镇卫生院1608名临床护士及238名护理管理者作为研究对象,自行设计问卷调查表,采用问卷星进行调查统计。结果共发放临床护士问卷1650份,回收有效问卷1608份,有效回收率为97.45%;向护理管理者发放问卷241份,回收有效问卷238份,有效回收率为98.76%。39.12%(629/1608)的临床护士患有下肢静脉曲张。不同医院等级、科室、年龄、专业技术职务、工龄、下肢静脉曲张家族史、夜班情况、生育情况、上班期间行走时间比较,差异均有统计学意义(P<0.05)。医院等级、科室、工龄、下肢静脉曲张家族史、工作强度、每天行走时间、站立时间是临床护士下肢静脉曲张发生的主要影响因素,差异均有统计学意义(P<0.05)。50.42%(120/238)的护理管理者未对护士进行下肢静脉曲张防范知识培训,68.07%(162/238)的护理管理者未向护士提供防护措施。结论该地区护士下肢静脉曲张患病率较高,护理管理者重视不够,应提高护理管理者的认知,积极采取合理的综合性防护措施,从而降低护士下肢静脉曲张发生率,促进护士职业健康。
Objective To investigate the status quo of varicose veins in lower limbs of clinical nurses in a area of Chongqing,and to analyze its influencing factors so as to provide a basis for further intervention and improvement of protection level.Methods The convenient sampling method was used to select 1608 clinical nurses and 238 nursing managers from 1 grade A class 3 general hospital,7 second class hospitals,15 township hospitals in a area of Chongqing from June to December 2019 as the study subjects.The questionnaire was designed by ourselves and the questionnaire stars were used for conducting the investigation and statistics.Results A total of 1650 questionnaires were distributed,1608 effective questionnaires were recovered with an effective recovery rate of 97.45%;241 questionnaires were distributed to the nursing managers,238 effective questionnaires were recovered with an effective recovery rate of 98.76%.39.12%(629/1608)nurses had varicose veins of the lower extremities.There were statistically significant differences among different hospital grades,departments,ages,professional titles,nursing ages,family history of varicose veins,night shift,childbearing and walking time during work(P<0.05).The hospital grade,department,working age,family history of varicose veins,working intensity,daily walking time and standing time were the main influencing factors of varicose veins occurrence in lower limbs(P<0.05).50.42%(120/238)of the nursing managers did not train the nurses by the prevention knowledge of varicose veins,68.07%(162/238)of the nursing managers did not provide the protective measures to nurses.Conclusion The incidence rate of varicose veins of the lower limbs in this area is high,and the nursing administrators pay little attention to it.The cognition of the nursing managers should be increased to take the reasonable and comprehensive protective measures,thus to reduce the incidence rate of varicose veins of the lower limbs and promote the occupation health of nurses.
