PCR荧光探针法、乳胶法及培养法检测生殖道分泌物中沙眼衣原体、淋球菌和解脲支原体的优劣分析 被引量:8

摘要 目的比较PCR荧光探针法、乳胶法及培养法对生殖道分泌物中沙眼衣原体、淋球菌和解脲支原体的检出率差异,评价不同检测方法对不同性病病原体的检测意义。方法收集性病门诊患者的泌尿生殖道分泌物,采用PCR荧光探针法检测沙眼衣原体、淋球菌和解脲支原体,同时采用乳胶法检测沙眼衣原体,培养法检测淋球菌和解脲支原体,并将PCR荧光探针法的检测结果与乳胶法和培养法进行比较。结果587例标本同时完成沙眼衣原体PCR荧光探针法和乳胶法检测,PCR荧光探针法检测阳性率为11.9%(70/587)显著高于乳胶法(2.9%,17/587),差异具有统计学意义(P<0.001);609例标本同时完成淋球菌PCR荧光探针法和培养法检测,PCR荧光探针法检测阳性率为9.0%(55/609)略高于培养法(7.4%,45/609),差异无统计学意义(P>0.05);1022例标本同时完成解脲支原体PCR荧光探针法和培养法检测,PCR荧光探针法检测阳性率为44.9%(459/1022)高于培养法(40.0%,409/1022),差异具有统计学意义(P<0.05)。结论与乳胶法相比,PCR荧光探针法能显著提高沙眼衣原体的阳性检出率,有助于临床的早期诊断和及时治疗;相较于培养法,PCR荧光探针法能够提高解脲支原体的阳性检出率,对淋球菌的检出率与培养法相近。PCR探针法具有特异性高、敏感性强、操作简便等优点,培养法检测可以进行病原菌耐药性分析,指导临床合理用药,对于考虑淋球菌或解脲支原体感染的患者,可选择PCR探针法和培养法同时检测。 Objective To compare the efficiency of PCR fluorescent probe method,ulture method and latex method in screening the positive rate of Chlamydia trachomatis,Neisseria gonorrhoeae and Ureaplasma urealyticum in urogenital secretions.Methods Urogenital secretion samples were collected from patients in STD clinics.PCR fluorescent probe method and two control methods(latex methodand culture method)were applied to detect Chlamydia trachomatis,Neisseria gonorrhoeae and Ureaplasma urealyticum.The results of PCR fluorescent probe method were compared with those of latex method and culture method.Results Both PCR fluorescence probe method and latex method were used to detect C.trachomatis in 587 specimens.The positive rate of the former(11.9%,70/587),was significantly higher than that of the latter(2.9%,17/587)(P<0.01).Both PCR fluorescence probe method and culture method were used to detect N.gonorrhoeae in 609 specimens.The positive rate of the former was slightly higher than that of the latter(9.0%,55/609 VS 7.4%,45/609)(P>0.05).Both PCR fluorescence probe method and culture method were applied to detect U.urealyticum in 1022 specimens.The positive rate of the former(44.9%,459/1022)was significantly higher than that of the latter(40.0%,409/1022)(P<0.05).Conclusion Compared with the latex method,PCR fluorescence probe method can significantly improve the positive detection rate of Chlamydia trachomatis in urinogenital secretions,which may help for early diagnosis and prompt treatment.Compared with the culture method,the PCR fluorescent probe method can increase the positive detection rate of Ureaplasma urealyticum,and the positive detection rate of Neisseria gonorrhoeae is similar to the culture method.PCR probe method has the advantages of high specificity,strong sensitivity and simple operation.culture method can be used to analyze the drug resistance of pathogenic bacteria to guide clinical use of drug.For patients considering Neisseria gonrrhoeae or Ureaplasma urealyticum infection,PCR probe method and culture method can be selected for simultaneous detection.
