摘要
目的观察三黄忍冬藤汤促进子宫瘢痕憩室患者术后瘢痕修复的临床疗效。方法选取2017年2月-2018年12月浙江中医药大学附属第一医院收治的50例子宫瘢痕憩室患者为研究对象,随机分为中药组和对照组,每组各25例。所有患者均给予宫腹腔镜联合手术进行瘢痕憩室修补。术后中药组患者给予三黄忍冬藤汤治疗持续3个月,对照组不予药物干预,随访3个月。比较两组患者治疗前后月经量、行经天数、瘢痕肌层厚度、症状积分及临床疗效。结果两组患者术后行经天数、月经量及症状积分均较前显著降低,瘢痕肌层厚度较治疗前显著升高,差异均有统计学意义(均P<0.05)。且治疗后,两组患者行经天数[(6.8±1.8)d、(5.9±1.3)d]、瘢痕肌层厚度[(7.9±1.2)mm、(8.9±1.4)mm]及症状积分[(4.5±1.0)分、(3.9±0.9)分]比较,差异均有统计学意义(t=2.240、-2.640、2.340,均P<0.05)。中药组患者治疗有效率(96.00%)高于对照组(88.00%),但差异无统计学意义(χ^(2)=2.500,P>0.05)。结论宫腹腔镜联合术对于子宫瘢痕憩室患者具有良好的治疗效果,可明显改善患者临床症状,提高生活质量。术后给予三黄忍冬藤汤益气清热利湿,凉血化瘀止血治疗,可以进一步缩短患者行经天数,增加瘢痕肌层厚度,改善临床症状。
Objective To observe the effect of San Huang Ren Dong Teng decoctionon scar repair in patients with cesarean scar diverticulum aftersurgey.Methods A total of 50 patients with uterine scar diverticulum admitted to the First Affiliated Hospital of Zhejiang Chinese Medicine University from February 2017 to December 2018 were selected as the research subjects and randomly divided into the traditional Chinese medicine group and the control group,with 25 patients in each group.All patients received combined laparoscopic surgery to repair scar diverticulum.After operation,patients in the traditional Chinese medicine group were treated with San Huang Ren Dong Teng decoction for 3 months,while patients in the control group were followed up for 3 months without drug intervention.Menstrual volume,menstrual duration,scar muscle thickness,symptom score and clinical efficacy were compared between the two groups before and after treatment.Results Postoperative menstrual days,menstrual volume and symptom scores in two groups were significantly lower than before,and the thickness of scar muscle was significantly higher than before,the differences were statistically significant(P<0.05).There were significant difference between the two groups after treatment in menstrual days[(6.8±1.8)d,(5.9±1.3)d],the thickness of scar muscle layer[(7.9±1.2)mm,(8.9±1.4)mm]and symptom score[(4.5±1.0)score,(3.9±0.9)score](t=2.240,-2.640,2.340,all P<0.05).The effective rate in the herbal group(96.00%)was higher than that in the control group(88.00%),but there was no statistically significant difference(χ^(2)=2.500,P>0.05).Conclusion Uterine laparoscopic combined surgery has a good therapeutic effect on patients with uterine scar diverticulum,which can significantly improve the clinical symptoms of patients and improve the quality of life.Postoperative treatment of San Huang Ren Dong Teng decoction for qi,heat and humidity,cooling blood,removing blood Stasis and Stopping Blood can further shorten the duration of menstruation,increase the thickness of scar muscle and improve clinical symptoms.
