摘要
目的探讨对翼状胬肉采取手术切除与自体角膜缘干细胞移植联合治疗后佩戴绷带镜的临床疗效。方法 50例翼状胬肉患者,以随机数字表法分为对照组(35眼)和观察组(36眼),各25例。两组均采用手术切除联合自体角膜缘干细胞移植治疗,观察组术后佩戴绷带镜,对照组不佩戴绷带镜。比较两组角膜创面上皮愈合时间、结膜充血水肿退去时间、并发症发生情况、临床疗效、复发情况。结果观察组角膜创面上皮愈合时间(7.14±2.18)d、结膜充血水肿退去时间(2.36±0.41)d均短于对照组的(11.84±2.34)、(3.98±0.54)d,差异有统计学意义(P<0.05)。观察组并发症发生率5.56%低于对照组的22.86%,差异有统计学意义(P<0.05)。观察组治疗总有效率94.44%高于对照组的74.29%,差异有统计学意义(P<0.05)。观察组疾病复发率与对照组比较,差异无统计学意义(P>0.05)。结论对翼状胬肉采取手术切除联合自体角膜缘干细胞移植治疗后,佩戴绷带镜可加快创面愈合,缩短结膜充血水肿退去时间,同时还能减少并发症发生,预后性较高,临床疗效确切,具有一定临床推广和应用价值。
Objective To investigate the clinical effects of wearing bandage contact lens after surgical resection combined with autologous stem cells transplantation of limbus corneae in the treatment of pterygium.Methods 50 patients with pterygium were randomly divided into a control group(35 involved eyes)and an observation group(36 involved eyes),with 25 patients in each group.Both groups were treated with surgical resection combined with autologous limbal stem cell transplantation.Patients in the observation group wore bandage contact lens after operation,while patients in the control group did not wear bandage contact lens.Healing time of corneal wound epithelium,regression time of conjunctival congestion and edema,incidence of complications,clinical efficacy,recurrence rate were compared between these two groups.Results The observation group had a healing time of corneal wound epithelium of(7.14±2.18)d and a regression time of conjunctival congestion and edema of(2.36±0.41)d,which were shorter than the corresponding(11.84±2.34)and(3.98±0.54)d in the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 5.56%,which was lower than 22.86%in the control group,and the difference was statistically significant(P<0.05).The overall clinical efficacy in the observation group was 94.44%,which was higher than 74.29%in the control group,and the difference was statistically significant(P<0.05).The difference in recurrence rate between the observation group and the control group was not statistically significant(P>0.05).Conclusion After applying surgical resection combined with autologous stem cells transplantation of limbus corneae in the treatment of pterygium,wearing bandage contact lens can accelerate wound healing and shorten regression time of conjunctival congestion and edema,while this can also reduce the incidence of complications,has a high prognosis and a certain clinical effect.Thus,it is worthy of clinical promotion and clinical application.
