摘要
目的探讨高频超声对手部各型软组织包块的诊断价值,提高超声诊断率。方法回顾性分析经我院收治的98例手部肿物的超声声像图特征,根据病理结果,将研究对象分为常见组和罕见组,比较2组超声检出率和诊断符合率,并以病理结果为金标准,分析超声鉴别诊断手部包块良恶性的敏感度、特异度及准确度。结果98例手部肿物中,超声检出率为100%,诊断符合率为81%(79/98)。常见组检出率为100%,诊断符合率为95%(75/79);罕见组检出率为100%,诊断符合率为21%(4/19)。以病理结果为金标准,超声鉴别诊断手部包块良恶性的敏感度、特异度及准确度分别为97%、75%、97%。结论高频超声对鉴别手部包块的良恶性具有较高的价值,是手部常见软组织包块的良好诊断手段,但目前对少见及罕见肿块的诊断率仍较低。
Objective To investigate the value of high-frequency ultrasound in diagnosing soft tissue masses of the hands with an attempt to improve the diagnostic yield of ultrasonography.Methods A retrospectively study was conducted on features of ultrasound imaging in 98 patients with hand masses admitted to our hospital.The ultrasound findings were compared with the pathology.The overall detection yield and diagnosis coincidence by ultrasound were calculated.According to pathology,the study subjects were divided into the common mass group and the rare mass group,and thereby the detection yield and diagnosis coincidence by ultrasound in the two groups were calculated.Using pathological findings as gold standard,the sensitivity,specificity and accuracy were calculated for ultrasound in distinguishing between benign and malignant hand masses.Results In 98 patients with hand masses,the overall ultrasound detection yield was 100%,and the overall diagnostic coincidence was 81%(79/98).After group assignment,the detection yield and the diagnostic coincidence were 100%and 95%(75/79)in the common mass group,compared with 100%and 21%(4/19),respectively,in the rare mass group.With pathological findings as gold standard,the sensitivity,specificity and accuracy of ultrasound in the differential diagnosis of benign and malignant hand masses were 97%,75%,and 97%,respectively.Conclusion High-frequency ultrasound is highly valuable in distinguishing between benign and malignant hand masses,and offers a good diagnostic tool for common soft tissue masses of the hands.Unfortunately,the diagnostic yield for uncommon or rare masses remains low.
