摘要 目的分析CT灌注成像(CTPI)对肺癌化疗患者的疗效评估价值。方法该研究对象为方便选取2018年7月—2020年7月间来院治疗的58例肺癌化疗患者,所有患者的化疗方案均为TP(紫杉醇+铂类药物)。根据化疗疗效分为A组47例(缓解组)和B组11例(未缓解组)。对比两组的灌注参数值。结果治疗后,A组的血流量(BF)为(65.18±4.68)mL/(min·100 g),B组为(58.81±4.15)mL/(min·100 g),差异有统计学意义(t=4.144,P<0.001);A组的灌注值为(14.98±0.67)mL/(min·100 g),B组为(14.03±0.62)mL/(min·100 g),差异有统计学意义(t=4.288,P<0.001);A组的血容量(BV)为(4.11±0.31)mL/100 g,B组为(4.60±0.44)mL/100 g,差异有统计学意义(t=4.342,P=0.001);A组的表面通透性(PS)为(15.42±4.33)mL/100 g,B组为(12.19±3.81)mL/100 g,差异有统计学意义(t=2.273,P=0.027);A组的强化幅度(18.64±2.84)HU,B组为(21.77±2.91)HU,差异有统计学意义(t=3.276,P=0.002);A组的平均通过时间(MTT)为(5.72±0.95)s,B组为(5.11±0.34)s,差异有统计学意义(t=2.086,P=0.042)。结论为肺癌化疗患者行CTPI检查可评估疗效,反映肿瘤内血供情况,定量分析血流灌注的具体改变,进而优化化疗方案。 Objective To analyze the value of CT perfusion imaging(CTPI)in evaluating the efficacy of chemotherapy for lung cancer patients.Methods The researceh object was to facilitate the selection of 58 patients with lung cancer chemotherapy who came to the hospital for treatment between July 2018 and July 2020.The chemotherapy regimen for all patients was TP(paclitaxel+platinum drugs).According to the efficacy of chemotherapy,they were divided into 47 cases in group A(remission group)and 11 cases in group B(non-remission group).Compared the perfusion parameter values of the two groups.Results After treatment,the blood flow(BF)of group A was(65.18±4.68)mL/(min·100 g),group B was(58.81±4.15)mL/(min·100 g),the difference was statistically significant(t=4.144,P<0.001);the perfusion value of group A was(14.98±0.67)mL/(min·100 g),group B was(14.03±0.62)mL/(min·100 g),the difference was statistically significant(t=4.288,P<0.001);blood volume(BV)was(4.11±0.31)mL/100 g,group B was(4.60±0.44)mL/100 g,the difference was statistically significant(t=4.342,P=0.001);the surface permeability of group A(PS)was(15.42±4.33)mL/100 g,group B was(12.19±3.81)mL/100 g,the difference was statistically significant(t=2.273,P=0.027);the enhancement range of group A was(18.64±2.84)HU,group B was(21.77±2.91)HU,the difference was statistically significant(t=3.276,P=0.002);the average transit time(MTT)of group A was(5.72±0.95)s,group B was(5.11±0.34)s,the difference was statistically significant(t=2.086,P=0.042).Conclusion CTPI examination for patients with lung cancer chemotherapy can evaluate the efficacy,reflect the blood supply in the tumor,quantitatively analyze the specific changes of blood perfusion,and then optimize the chemotherapy regimen.
