摘要 目的分析CT诊断颅脑肿瘤的临床应用价值。方法80例疑似颅脑肿瘤患者为研究对象,均行CT检查。以病理诊断为金标准,观察CT诊断颅脑肿瘤的敏感性、特异性、准确性。比较不同颅脑肿瘤分型患者肿瘤组织与正常侧组织的增强高度、对比剂通过时间、峰值时间、局部脑血流量、局部脑血容量。结果80例疑似颅脑肿瘤患者,经病理诊断确诊颅脑肿瘤72例,经CT诊断确诊颅脑肿瘤74例。CT诊断颅脑肿瘤的敏感性为98.61%(71/72)、特异性为62.50%(5/8)、准确性为95.00%(76/80)。经病理诊断,确诊脑膜瘤24例、脑转移瘤26例、脑胶质瘤22例,利用CT均可准确分辨。经CT检查显示,脑膜瘤、脑转移瘤、脑胶质瘤患者的肿瘤组织与正常侧组织的增强高度、峰值时间、局部脑血流量、局部脑血容量比较,差异具有统计学意义(P<0.05);肿瘤组织与正常侧组织的对比剂通过时间比较,差异无统计学意义(P>0.05)。脑膜瘤患者肿瘤组织的增强高度、局部脑血流量、局部脑血容量高于脑胶质瘤及脑转移瘤患者的肿瘤组织,脑胶质瘤患者肿瘤组织的增强高度、局部脑血流量、局部脑血容量高于脑转移瘤患者的肿瘤组织,差异具有统计学意义(P<0.05);脑膜瘤、脑转移瘤、脑胶质瘤患者肿瘤组织的对比剂通过时间、峰值时间两两比较,差异无统计学意义(P>0.05)。脑膜瘤、脑转移瘤、脑胶质瘤患者正常侧组织的增强高度、对比剂通过时间、峰值时间、局部脑血流量、局部脑血容量两两比较,差异无统计学意义(P>0.05)。结论CT诊断具有操作方便、准确性高、无创伤性的优势,将其应用于颅脑肿瘤患者的诊断中,敏感性、特异性、准确性均较高,具有临床推广价值。 Objective To analyze the clinical value of CT in the diagnosis of brain tumor.Methods There were 80 patients with suspected brain tumor as study subjects,and all received CT examination.With pathological diagnosis as the gold standard,the sensitivity,specificity and accuracy of CT in the diagnosis of brain tumors were observed.The enhancement height,contrast-media passage time,peak time,local cerebral blood flow,and local cerebral blood volume were compared between tumor tissue and normal tissue in patients with different brain tumor types.Results Of the 80 patients with suspected brain tumors,72 cases were diagnosed by pathology and 74 cases were diagnosed by CT.The sensitivity,specificity and accuracy of CT in the diagnosis of brain tumors were 98.61%(71/72),62.50%(5/8)and 95.00%(76/80).By pathological diagnosis,24 cases of meningioma,26 cases of brain metastases and 22 cases of glioma were confirmed,which could be accurately distinguished by CT.CT examination showed that there were statistically significant differences in enhancement height,peak time,local cerebral blood flow and local cerebral blood volume between tumor tissue and normal tissue in patients with meningioma,brain metastasis and glioma(P<0.05).There was no statistically significant difference in the passage time of contrast agent between tumor tissue and normal tissue(P>0.05).The enhancement height,local cerebral blood flow and local cerebral blood volume of tumor tissue in meningioma patients were higher than those of tumor tissue in glioma and brain metastasis patients,and patients with glioma were higher than those with brain metastases.All the difference was statistically significant(P<0.05).There was no statistically significant difference in the contrast-media passage time and peak time of the tumor tissues in patients with meningioma,brain metastasis,and glioma(P>0.05).There was no statistically significant difference in the enhancement height,contrast-media passage time,peak time,local cerebral blood flow,and local cerebral blood volume of the normal tissues in patients with meningioma,brain metastasis,and glioma(P>0.05).Conclusion CT diagnosis has the advantages of convenient operation,high accuracy and no trauma.It has high sensitivity,specificity and accuracy when it is applied to the diagnosis of brain tumor patients,which has the value of clinical promotion.
