血浆肾素活性和醛固酮及脂肪酶在原发性高血压患者中的检测意义 被引量:4

摘要 目的探究血浆肾素活性、醛固酮、内皮脂肪酶在原发性高血压患者中的检测意义。方法选取2018年1月至2019年7月期间我院接诊的原发性高血压患者158例,纳入观察组,根据高血压病情分级分为轻度组、中度组、重度组分别37、78、43例,所有患者实施降压治疗,观察3个月。另选取时期我院健康体检者50例,纳入对照组。分别比较观察组与对照组,不同高血压分级患者及原发性高血压患者治疗前后血浆肾素活性、醛固酮、内皮脂肪酶、收缩压、舒张压、高血压病情分度,采用Spearman相关性分析收缩压、舒张压与血浆肾素活性、醛固酮、内皮脂肪酶水平的相关性。结果(1)观察组与对照组、不同高血压分级患者血浆肾素活性、醛固酮、内皮脂肪酶水平比较,观察组高于对照组,重度组高于中度组、轻度组,中度组高于轻度组;(2)观察组治疗前后比较,血浆肾素活性、醛固酮、内皮脂肪酶均较治疗前升高,收缩压、舒张压均较治疗前降低,差异均有统计学意义(P<0.05);(3)Spearman相关性分析结果显示,观察组患者收缩压、舒张压与血浆肾素活性、醛固酮、内皮脂肪酶水平均呈正相关性(P<0.05)。结论原发性高血压患者血浆肾素活性、醛固酮、内皮脂肪酶高于正常水平,且与患者血压存在一定相关性,经降压治疗后有所改善,可作为病情判断及预后评价的参考指标。 Objective To explore the detection significance of plasma renin activity,aldosterone and endothelial lipase in patients with essential hypertension.Methods 158 patients with essential hypertension admitted to our hospital from January 2018 to July 2019 were selected and included in observation group.According to the hypertension classification,they were divided into mild group(37 cases),moderate group(78 cases),and severe group(43 cases).All patients were treated with antihypertensive treatment and observed for 3 months.Another 50 cases of healthy people with physical examination in our hospital during the same period were included in control group.The levels of plasma renin activity,aldosterone,endothelial lipase,systolic and diastolic blood pressure,and degree of hypertension before and after treatment were compared between observation group and control group and among patients with different hypertension classification and patients with essential hypertension.Spearman correlation analysis was used to analyze the correlation between systolic blood pressure and diastolic blood pressure and levels of plasma renin activity,aldosterone and endothelial lipase.Results Comparison of levels of plasma renin activity,aldosterone and endothelial lipase between observation group and control group and among patients with different blood pressure grades showed that the levels in observation group were higher than those in control group,and the levels in severe group were higher than in mild group and moderate group,and those in moderate group were higher than in mild group(P<0.05).The levels of plasma renin activity,aldosterone,and endothelial lipase in observation group after treatment were increased compared to before treatment while the systolic blood pressure and diastolic blood pressure were decreased compared to before treatment(all P<0.05).Spearman correlation analysis showed that the systolic blood pressure and diastolic blood pressure were positively correlated with levels of plasma renin activity,aldosterone,and endothelial lipase in observation group(all P<0.05).Conclusion The plasma renin activity,aldosterone,and endothelial lipase in patients with essential hypertension are higher than normal levels,and there is a certain correlation with the blood pressure of patients.After antihypertensive treatment,the levels have been improved,and can be used as reference indexes for disease judgment and prognosis evaluation.
