阴道微生态变化与宫颈高级别鳞状上皮内病变的相关性研究 被引量:5

摘要 目的对比分析阴道微生态变化与宫颈高级别鳞状上皮内病变发病的相关性。方法收集宫颈高级别鳞状上皮内病变者117例为研究组,同期门诊就诊行宫颈人乳头瘤病毒(HPV)及宫颈液基细胞学(TCT)检测均阴性的患者33例作为对照组,对比分析2组阴道微环境变化及代谢产物、酶的改变。结果研究组患者中过氧化氢阳性表达率80.34%、唾液酸苷酶阳性表达率34.19%、杂菌阳性表达率40.17%、线索细胞阳性表达率43.59%、清洁度Ⅱ、Ⅲ~Ⅳ度的表达率分别为52.14%、47.89%;白细胞脂酶阳性表达率37.61%、凝固酶阳性表达率19.66%、滴虫阳性表达率26.50%、白细胞/脓细胞比值中0~5、5~30表达率分别为73.50%、26.50%。2组相比过氧化氢阳性、唾液酸苷酶阳性、杂菌阳性、线索细胞阳性、清洁度差异有统计学意义(P<0.05),而白细胞脂酶阳性、凝固酶阳性、滴虫阳性、白细胞/脓细胞比值差异无统计学意义(P>0.05)。研究组中单纯性阴道炎表达率43.59%,混合性的阴道炎11.11%,乳酸杆菌功能下降表达率28.21%,白细胞酯酶阳性表达率4.27%,2组相比微生态结果中单纯性阴道炎、混合性阴道炎、乳酸菌功能下降差异具有统计学意义(P<0.05),白细胞酯酶阳性差异无统计学意义(P>0.05)。结论阴道微环境变化与宫颈高级别鳞状上皮内病变的发生密切相关。 Objective To analyze the correlation between the changes of vaginal microecology and the incidence of high-grade squamous intraepithelial(HSIL).Methods A total of 117 patients with cervical high-grade squamous intraepithelial lesions(HSIL)confirmed by pathology were collected.Meanwhile,cervical human papilloma virus(HPV)and Thinprep cytology test were performed in our gynecological clinic.The changes of vaginal microenvironment,metabolites and enzymes were compared between the two groups.Results The vaginal microbial metabolites and enzyme changes comparison:in the high-grade cervical intraepithelial neoplasia group,the positive expression rate of hydrogen peroxide was 80.34%;the positive expression rate of sialidase was 34.19%;and the positive expression rate of miscellaneous bacteria was 40.17%;the positive expression rate of clue cells was 43.59%;the expression rates of cleanliness II and III-IV were 52.14%and 47.89%,respectively.The positive rates of lipase,coagulase,trichomonas were respectively 37.61%,19.66%,26.50%,73.50%and 26.50%.There were significant differences in hydrogen peroxide positive,sialidase positive,mixed bacteria positive,clue cell positive and cleanliness between the two groups(P<0.05),but there were no significant differences in leukocyte lipase positive,coagulase positive,trichomonas positive and leukocyte/pyogenic cell ratio between the two groups(P>0.05).Comparison of vaginal microecological status:in high-grade cervical intraepithelial neoplasia group,the expression rate of simple vaginitis was 43.59%;mixed vaginitis was 11.11%;the expression rate of Lactobacillus function decreased was 28.21%;the positive expression rate of leukocyte esterase was 4.27%.Compared with the two groups:in the results of microecology,there were significant differences in the function of simple vaginitis,mixed vaginitis and lactic acid bacteria(P<0.05),but there was no significant difference in the positive rate of leukocyte esterase(P>0.05).Conclusion Vaginal microenvironment are closely related to the occurrence of cervical high-grade squamous intraepithelial lesions.
