摘要
目的:观察参黄消瘿汤结合左甲状腺素钠对气阴两虚型桥本甲状腺炎患者疗效、血清炎症因子影响。方法:研究纳入83例气阴两虚型桥本甲状腺炎患者,由我院2018年4月~2020年1月收治,以随机数字表法将其分为两组,对照组患者(41例)左甲状腺素钠治疗,观察组患者(42例)参黄消瘿汤结合左甲状腺素钠治疗,比较两组患者临床治疗效果、治疗前后两组患者中医症状(暴躁易怒、咽干口燥、自汗盗汗、气短懒言、烦渴欲饮、苔少而干等)积分变化、治疗前后两组患者甲状腺体积变化、治疗前后两组患者血清白介素-6(IL-6)、白介素-12(IL-12)、肿瘤坏死因子-α(TNF-α)水平变化、不良反应情况。结果:观察组患者治疗总有效率(95.24%)高于对照组(75.61%),P<0.05;治疗前,两组患者暴躁易怒、咽干口燥、自汗盗汗、气短懒言、烦渴欲饮、苔少而干等中医症状积分,甲状腺体积、IL-6及IL-12、TNF-α水平比较,P>0.05,治疗后各组患者暴躁易怒、咽干口燥、自汗盗汗、气短懒言、烦渴欲饮、苔少而干等中医症状积分,甲状腺体积、IL-6及IL-12、TNF-α水平均改善,观察组患者治疗后暴躁易怒、咽干口燥、自汗盗汗、气短懒言、烦渴欲饮、苔少而干等中医症状积分,甲状腺体积、IL-6及IL-12、TNF-α水平均优于对照组,P<0.05;两组治疗均未见不良反应,肝肾功能均无影响,P>0.05。结论:参黄消瘿汤结合左甲状腺素钠治疗气阴两虚型桥本甲状腺炎临床疗效佳,无不良反应,安全性高。
Objective:To explore the effect of Shenhuang Xiaoying Decoction combined with Levothyroxine on serum inflammatory factors foRHashimoto's thyroiditis with Qi-Yin deficiency.Methods:In this study,83 patients with hashimoto's thyroiditis with Qi-Yin deficiency were included and admitted to ouRhospital from April 2018 to January 2020.They were divided into two groups by random numbeRtable method.Patients in the controlled group(41 cases)were treated with Levothyroxine,while patients in the observation group(42 cases)were treated with Shenhuang Xiaoying Decoction combined with Levothyroxine.Compared two groups of patients with clinical therapeutic effect of traditional Chinese medicine(TCM),the two groups of patients before and afteRthe treatment,the symptoms(hot-tempered,pharynx dry mouth dryness,shortness of breath,spontaneous sweating night sweats thirsting lazy speech,vexed,moss and less dry,etc.)changes,two groups of patients with thyroid volume change before and afteRthe treatment,the two groups of patients before and afteRthe treatment of free triiodothyronine(FT3)and free teraiodothyronine(FT4),thyroid stimulating hormone(TSH)level change,two groups of patients before and afteRtreatment serum interleukin-6(IL-6),interleukin-12(IL-12),tumoRnecrosis factoRalpha(TNF-alpha)level changes,adverse reactions.Results:The total effective rate(95.24%)in the observation group was higheRthan that in the controlled group(75.61%),P<0.05.Before the treatment,two groups of patients with irritability,dry throat,dry mouth,shortness of breath,spontaneous sweating night sweats thirsting lazy speech,vexed,moss and TCM symptoms such as dry less integral,thyroid volume,FT3,FT4,TSH,IL-6 and IL-12,TNF-alpha level comparison,P>0.05,groups of patients afteRtreatment of irritability,dry throat,dry mouth,shortness of breath,spontaneous sweating night sweats thirsting lazy speech,vexed,moss and TCM symptoms such as dry less integral,thyroid volume,FT3,FT4,TSH,IL-6 and IL-12,to improve the level of TNF-alpha.AfteRtreatment,the scores of TCM symptoms,thyroid volume,FT3,FT4,TSH,IL-6,IL-12 and TNF-alpha in the observation group were all betteRthan those in the controlled group(P<0.05).No adverse reactions were observed in both groups,and liveRand renal function were not affected(P>0.05).Conclusion:Shenhuang Xiaoying Decoction combined with Levothyroxine Sodium has a good clinical effect on hashimoto's thyroiditis with Qi-Yin deficiency.
