孕前补硒对单纯甲状腺过氧化物酶抗体阳性孕妇妊娠结局的影响

The influence of selenium supplementation before pregnancy on pregnancy outcome in pregnant women with thyroid peroxidase autoantibodies

摘要 目的探讨孕前补硒对单纯甲状腺过氧化物酶抗体阳性孕妇妊娠结局的影响。方法前瞻性选取2019年6月~12月于首都医科大学大兴教学医院内分泌科就诊的备孕妇女(甲状腺过氧化物酶抗体阳性)100例,随机数表法分为观察组(50例)和对照组(50例)。观察组口服硒酵母胶囊,对照组不予治疗,定期监测甲状腺功能、甲状腺过氧化物酶抗体(TPOAb),并统计两组流产率及活产率。结果观察组TPOAb水平在补硒后较治疗前明显下降,差异有统计学意义(P<0.05)。孕后观察组TPOAb水平、TSH水平明显低于对照组,差异有统计学意义(P<0.05)。对照组孕24周时,FT3及FT4均较孕前下降,且均明显低于观察组,差异有统计学意义(P<0.05)。观察组流产率明显低于对照组,活产率高于对照组,差异有统计学意义(P<0.05)。结论孕前补硒能有效降低TPOAb水平,减少孕后甲状腺功能异常发生,降低流产率,提高活产率。 Objective To investigate the influence of selenium supplementation before pregnancy on pregnancy outcome in pregnant women with thyroid peroxidase antibody.Methods 100 women preparing to become pregnant(with thyroid peroxidase antibody)were selected,who visited the Department of Endocrinology,Daxing Teaching Hospital,Capital Medical University from June 2019 to December 2019.100 women were Randomly divided into treatment group(50 cases)and control group(50 cases).The treatment group was given selenium yeast capsules orally,while the control group was not treated.The thyroid function and thyroid peroxidase antibody(TPOAb)were monitored regularly,and the abortion rate and live birth rate of the two groups were counted.Results The level of TPOAb in the treatment group was significantly lower after selenium supplementation compared with that before treatment(P<0.05).The level of TPOAb in the treatment group was still significantly lower than control group after pregnancy(P<0.05).The level of TSH in the treatment group was significantly lower than the control group after pregnancy(P<0.05).At 24 weeks of pregnancy,FT3 and FT4 in the control group were lower than those before pregnancy(P<0.05),and they were significantly lower than the treatment group(P<0.05).The abortion rate of the treatment group was significantly lower than the control group(P<0.05).The live birth rate of the treatment group was significantly higher than the control group(P<0.05).Conclusion Selenium supplementation before pregnancy can effectively reduce the level of thyroid antibody,reduce the incidence of thyroid dysfunction after pregnancy,reduce the rate of early abortion and improve the rate of live birth.
