摘要
介绍王坤根教授从五脏论治黄褐斑的经验。王坤根教授认为,黄褐斑的病机是五脏功能失调,导致气血运行异常,面部肌肤失于濡养,日久出现色素性沉着。主张从五脏论治:从肝论治,平衡阴阳,气血同治;从脾论治,健脾益气,养血美颜;从肾论治,精血同源,补肾填精;从肺论治,调畅气机,表病里治;从心论治,宁心安神,养血活血。病程较长,且易反复发作,一则以主证为要,兼顾他证,以此选方用药;二则对患者精神情志影响较大,疏肝理气应贯穿始终。总体治法为:五脏论治,气血同治,理气为要,以血为本,养颜消斑。
This paper introduces the experience of Prof.WANG Kungen in treating chloasma from the five-zang organs based on syndrome differentiation.Prof.WANG believes that the pathogenesis of chloasma is dysfunction of the five-zang organs,leading to abnormal qi movement and blood,under-nourishment of facial skin and over time resulting in pigmentation.It is advocated to treat the disease from the five-zang organs based on syndrome differentiation:to treat from the liver:balance yin and yang,and treat qi and blood together;to treat from the spleen:strengthen the spleen and tonify qi,nourish blood and beautify the face;to treat from the kidney:essence and blood are from the same source,so it is to nourish the kidney and supplement essence;to treat from the lung:regulate qi movement,and treat the exterior disease from the interior;to treat from the heart:rest the heart and calm the mind,and nourish and invigorate the blood.The course of the disease is long,and it is easy to have repeated attacks.For one thing,when prescribing and medicating,the main syndromes should be given priority and the other syndromes should be taken into account.For another thing,the disease has a great influence on the patient’s mental and emotional state,so soothing the liver and regulating qi should run through the whole process.The overall treatment is as follows:to treat the five-zang organs,treat qi and blood together,regulate qi as the priority,take blood as the basis,nourish the face and eliminate spots.
