摘要
目的观察参附注射液治疗慢性心力衰竭(心肾阳虚证)的疗效及其对血清N-末端B型脑钠肽前体(NT-proBNP)水平的影响并探讨其作用机制。方法120例患者按随机数字表法分为对照组和观察组各60例,均按照指南行常规治疗。对照组在西医规范化治疗基础上加用安慰剂治疗;观察组在对照组基础上加参附注射液治疗。两组连续治疗2周。比较两组心肾阳虚证主症评分、心功能、临床疗效以及血清NT-proBNP水平。结果两组治疗后,患者心肾阳虚证主症(心悸、短气乏力、身寒肢冷)评分与治疗前比较,均明显降低(均P<0.01),且观察组均明显低于对照组(均P<0.01)。两组治疗后左心室舒张末期内径(LVEDD)、心室收缩末期内径(LVESD)以及Lee氏心衰计分与治疗前比较,均降低,而LVEF、6 min步行试验(MWT)则均增加(均P<0.01);且观察组治疗后患者LVEDD、LVESD以及Lee氏心衰计分均低于对照组,LVEF、6 MWT均高于对照组(均P<0.01)。观察组的总有效率为91.67%,明显高于对照组的76.27%(P<0.05)。两组治疗后,患者血清NT-proBNP水平与治疗前比较降低(P<0.01),观察组低于对照组(P<0.01)。结论在常规内科治疗基础上,参附注射液治疗慢性心力衰竭(心肾阳虚证)的疗效显著,且能有效下调血清NT-proBNP水平。
Objective:To investigate the efficacy of Shenfu Injection in treating chronic heart failure with syndrome of yang deficiency of heart and kidney and its effect on serum level of N-terminal B-type natriuretic peptide precursor(NT-proBNP).Methods:One hundred and twenty chronic heart failure were selected according to random number table method,and the 120 patients were divided into the control group and observation group,60 cases in each group.Both groups were given routine treatment according to the guidelines;the control group was treated with placebo on the basis of standardized treatment of western medicine,and the observation group was given Shenfu Injection on the basis of control group.The two groups were treated for 2 weeks.Scores of main symptoms,heart function,the clinical efficacy,serum levels of NT-proBNP of the two groups were compared.Results:After treatment,the main symptom scores,left ventricular end-diastolic diameter(LVEDD),left ventricular endsystolic diameter(LVESD),and Lee′s score of the observation group were significantly lower,while the left ventricular ejection fraction(LVEF)and 6-min walk test(6 MWT)were significantly higher than those of the control group(P<0.01).The total effective rate of the observation group was 91.67%,which was significantly higher than 76.27%of the control group(P<0.05).After treatment,the serum level of NT-proBNP in the observation group were significantly lower than those in the control group(P<0.01).Conclusion:Shenfu Injection in treating chronic heart failure with syndrome of yang deficiency of heart and kidney has an significant efficacy,and can down-regulate serum level of NT-proBNP.
