隐形矫治配合Ⅱ类牵引时附件高度对尖牙稳定及矫治器贴合的影响 被引量:1

摘要 目的:研究隐形矫治器配合Ⅱ类牵引时,垂直矩形附件高度对尖牙稳定及矫治器贴合的影响,为临床附件设计提供参考。方法:建立含有牙槽骨-牙周膜-上牙列-隐形矫治器-附件-树脂扣的有限元模型,其中附件设置3种高度(分别为3、4、5 mm),模拟上颌尖牙受到1 N的颌间牵引力,读取施力瞬间尖牙位移、牙周膜应力分布,并计算尖牙与矫治器各个定位点的相对位移以评价尖牙区矫治器贴合情况。结果:对尖牙施加牵引力后,随着附件高度增加,尖牙位移及牙周膜应力先减小后增大,表现为4 mm附件<5 mm附件<3 mm附件;4 mm附件尖牙与矫治器之间的龈方相对位移最小;5 mm附件尖牙与矫治器之间的牙合方相对位移最小。结论:隐形矫治器配合Ⅱ类牵引时,适当增大垂直附件高度有利于尖牙稳定及尖牙区矫治器贴合,但应结合尖牙临床冠长度,选择最佳高度。 Aim:To study the effects of vertical rectangular attachments of different heights on the stability of the maxillary canine and the fit of aligners during clear aligners combined with classⅡelastics,so as to provide reference for the design of clinical attachments.Methods:A finite element model was established containing the alveolar bone-periodontal ligament-maxillary teeth-peridontal-aligner-attachments-resin button.The attachments were set at 3 different heights(3,4,and 5 mm,respectively).To simulate the maxillary canine to be subjected to 1 N intermaxillary traction force,read the canine displacement,the periodontal ligament stress,and calculate the relative displacement between the canine and the appliance at each positioning point to evaluate the fit of aligners.Results:After tractive force was applied to the canine,with the increase of the attachment height,the displacement of the canine and the stress in the periodontal ligament first decreased and then increased,as shown in the 4 mm attachment<5 mm attachment<3 mm attachment;the relative displacement of the gingival side between the attachment and the aligners was minimal at the height of 4 mm;the relative displacement of the occlusal side between the attachment and the aligners was minimal at the height of 5 mm.Conclusion:When invisible aligners are combined with classⅡelastics,increasing the height of the vertical rectangle attachment is beneficial to stabilize the canine and correctional implement joint,but the clinical crowns size must be taken into account comprehensively to choose the best attachment.
