摘要
目的观察体外冲击波疗法(extracorporeal shock wave therapy,ESWT)在新兵训练后胫骨应力性骨损伤患者的治疗效果。方法对2018年10月—2019年10月在某新兵团治疗的40例胫骨应力性骨损伤战士,随机分为试验组(20例)和对照组(20例)。试验组使用发散式体外冲击波对损伤部位进行干预,对照组采用支具固定的方法。治疗后每4周复查两组战士DOMS评分、血清骨特异性碱性磷酸酶、每2周复查MRI及超声检查,进行统计学分析。结果两组相同时间DOMS评分试验组疼痛消失时间明显早于对照组,差异有统计学意义(P<0.05)。血清骨特异性碱性磷酸酶(serum bone-specific alkaline phosphatase,sBAP),治疗后4周治疗组与对照组有显著统计学意义(P>0.05)。MRI及超声检查显示骨膜及骨髓水肿吸收实验组明显好于对照组。结论体外冲击波(extracorporeal shock wave,ESW)在胫骨应力性骨损伤治疗上能够缓解疼痛,促进骨肌系统水肿的吸收,加速骨折的愈合,明显优于传统的治疗方法。
Objective To observe the effect of extracorporeal shock wave therapy in patients with tibia stress bone inju⁃ry after recruit training.Methods From October 2018 to October 2019,40 soldiers with tibial stress bone injury who were treated in a new army group were randomly divided into a test group(20 cases)and a control group(20 cases).The experimental group used divergent extracorporeal shock waves to intervene on the injury site,and the control group used a brace fixation method.After treatment,the soldiers'DOMS score,serum bone-specific alkaline phosphataseev⁃ery 4 weeks,MRI and ultrasound were reviewed every 2 weeks for statistical analysis.Results The pain disappeared in the DOMS score test group at the same time between the two groups was significantly earlier than that in the control group,and the difference was statistically significant(P<0.05).Serum bone-specific alkaline phosphatase(sBAP),4 weeks after treatment,the treatment group and the control group were statistically significant(P>0.05).MRI and ultra⁃sonography showed that the absorption of periosteal and bone marrow edema in the experimental group was significant⁃ly better than that in the control group.Conclusion Extracorporeal shock wave(ESW)in the treatment of tibial stress bone injury can relieve pain,promote the absorption of musculoskeletal system edema,and accelerate the healing of fractures,which is significantly better than traditional treatment methods.Extracorporeal shock wave is a simple,safe,effective and non-invasive treatment method with a wide range of applications.
