摘要
目的研究高场磁共振回声不对称和最小二乘估算法水脂迭代分离序列技术(IDEAL-IQ)在急性非胆源性胰腺炎脂肪浸润中的评估作用,为急性非胆源性胰腺炎的诊断提供临床应用价值。方法回顾性分析2016年10月至2019年10月于本院就诊的急性非胆源性胰腺炎患者35例及同期招募的正常对照组35例,分别抽取肘静脉血检查血Amy、Lps、TC、TG,对APACHEⅡ进行分级,进行MRI T1W1、T2W1和IDEAL-IQ定量分析,比较两组脂肪分数值FF值、R2^(*)值差异及与实验室检查的相关性。结果对照组血清胰淀粉酶、脂肪酶、总胆固醇、三酰甘油及APACHEⅡ评分均较观察组低,差异均有统计学意义(P<0.05);对照组胰腺各部位及全胰腺FF值及R2^(*)值均较观察组低,差异均有统计学意义(P<0.05);两组FF值和R2^(*)值与Amy、Lps、TC、TG呈中度正相关,与APACHEⅡ呈弱相关。结论高场磁共振水-脂模型定量分析能够对急性非胆源性胰腺炎患者胰腺脂肪浸润进行定量分析,重复性好,结果可靠,无创安全,对临床诊断具有重要的指导价值。
Objective To explore the evaluation effect of high-field magnetic resonance iterative decomposition of water and fat with echo asymmetry and least squares estimation quantification sequence(IDEAL-IQ)in fat infiltration in acute non-biliary pancreatitis in order to provide value of clinical application for the diagnosis of acute non-biliary pancreatitis.Methods A retrospective analysis was conducted on 35 patients with acute non-biliary pancreatitis who were treated in our hospital from October 2016 to October 2019 and 35 normal controls who were recruited during the same period.The cubital venous blood was drawn for the detection of serum Amy,Lps,TC,and TG.APACHEⅡ scores were classified.Quantitative analyses of MRI T1W1,T2W1 and IDEAL-IQ were conducted.The fraction value(FF value),R2^(*) value,and correlation with laboratory examination were compared between the two groups.Results The serum Amy,Lps,TC,and TG and APACHEⅡ score of the control group were lower than those of the observation group,with statistically significant differences(P<0.05).The FF values and R2^(*) values of each location of pancreas and whole pancreas of the control group were lower than those of the observation group,with statistically significant differences(P<0.05).The FF values and R2^(*) values of the two groups were moderately positively correlated with serum Amy,Lps,TC,and TG,and were weakly correlated with APACHEⅡ.Conclusion The high-field magnetic resonance water and fat model quantitative analysis quantitatively analyzes the pancreatic fat infiltration in patients with acute non-biliary pancreatitis.It is non-invasive and safe,with good repeatability and reliable results,showing important guiding value for clinical diagnosis.
