cTTE与cTCD同步试验在卵圆孔未闭诊断中的应用 被引量:5

摘要 目的探讨经胸超声心动图声学造影(cTTE)联合经颅多普勒超声声学造影(cTCD)对卵圆孔未闭(PFO)的诊断价值。方法选取2020年1—12月河南科技大学第一附属医院接收的68例疑似为PFO的患者作为研究对象,入院后均接受cTTE与cTCD检查,并以经食管超声心动图(TEE)为“金标准”,比较cTTE、cTCD单独及联合诊断与TEE结果的一致性、准确度、灵敏度及特异度,分析cTTE联合cTCD对PFO的诊断价值。结果经TEE检查结果发现,68例患者中,PFO 60例,占88.24%;非PFO 8例,占比11.76%;经Kappa一致性检验,结果显示,cTTE诊断PFO结果和TEE结果的一致性一般(Kappa=0.449,P<0.001);cTCD诊断PFO结果和TEE结果的一致性一般(Kappa=0.506,P<0.001);cTTE联合cTCD诊断PFO结果和TEE结果的一致性较好(Kappa=0.775,P<0.001);cTTE联合cTCD诊断PFO的准确度、灵敏度均较cTTE、cTCD单独诊断高,差异有统计学意义(P<0.05);不同诊断方法特异度比较,差异无统计学意义(P>0.05)。结论cTTE联合cTCD在PFO诊断中的应用价值较高,具有较高的准确度及灵敏度,可为临床制定治疗方案提供重要指导。 Objective To evaluate the diagnostic value of contrast transthoracic echocardiography(cTTE)combined with transcranial Doppler contrast echocardiography(cTCD)in patent foramen ovale(PFO).Methods Sixty-eight patients with suspected PFO received by the First Affiliated Hospital of Henan University of Science and Technology from January 2020 to December 2020 were prospectively selected as the research subjects,cTTE and cTCD were performed after admission,and transesophageal echocardiography(TEE)was taken as the gold standard to compared the the consistency,accuracy,sensitivity and specificity of single and combined diagnosis of cTTE,cTCD and TEE results,the diagnostic value of cTTE combined with cTCD in PFO was analyzed.Results The results of TEE examination showed that there were 60 cases of PFO in 68 patients,accounting for 88.24%;there were 8 cases of non PFO,accounting for 11.76%;the results of Kappa consistency test showed that the consistency between result of cTTE in the diagnosis of PFO and TEE result was general(Kappa=0.449,P<0.001);the consistency between result of cTCD in the diagnosis of PFO and TEE result was general(Kappa=0.506,P<0.001);the consistency between result of cTTE combined with cTCD in the diagnosis of PFO and TEE result was good(Kappa=0.775,P<0.001);the accuracy and sensitivity of cTTE combined with cTCD in the diagnosis of PFO were higher than that of single diagnosis of cTTE and cTCD(P<0.05);there was no statistically difference in the specificity of different diagnostic methods(P>0.05).Conclusion cTTE combined with cTCD has high application value in the diagnosis of PFO,with high accuracy and sensitivity,which can provide important guidance for clinicians to make treatment regimens.
