摘要
目的观察术前乳腺癌组织Yes相关蛋白(Yes-associated protein,YAP)表达情况,探讨其与乳腺癌患者改良根治术后短期复发转移的相关性。方法213例乳腺癌患者均实施改良根治术,术前检测患者血清肿瘤标志物糖类抗原(carbohydrate antigen,CA)125、CA15-3、β2-微球蛋白(β2-microglobulin,β2-MG)、癌胚抗原(carcinoembryonic antigen,CEA)水平,采用免疫组织化学法检测乳腺癌组织YAP表达情况,术后随访1年,根据是否复发转移分为复发转移组50例和未复发转移组163例;比较2组患者临床资料、术前血清CA125、CA15-3、β2-MG、CEA水平及乳腺癌组织YAP表达情况,采用多因素logistic回归分析乳腺癌患者改良根治术后短期复发转移的影响因素。结果术后1年,213例乳腺癌患者复发转移50例,复发转移率为23.47%。复发转移组术前血清CA125[(37.38±4.26)u/mL]、CA15-3[(36.84±5.21)u/mL]、β2-MG[(2.61±0.27)mg/L]、CEA[(1.89±0.15)μg/L]水平及癌组织YAP阴性表达率(60.00%)高于未复发转移组[(34.72±4.81)u/mL、(34.32±5.17)u/mL、(2.43±0.24)mg/L、(1.80±0.14)μg/L、20.25%](P<0.05);术前血清CA125(OR=1.137,95%CI:1.045~1.238,P=0.003)、CA15-3(OR=1.104,95%CI:1.042~1.169,P=0.001)、β2-MG(OR=16.408,95%CI:3.677~73.211,P<0.001)、CEA(OR=18.695,95%CI:4.891~71.465,P<0.001)及YAP表达情况(OR=24.757,95%CI:1.758~348.738,P<0.001)是乳腺癌患者改良根治术后短期复发转移的影响因素。结论术前血清CA125、CA15-3、β2-MG、CEA水平增高及癌组织YAP阴性表达的乳腺癌患者改良根治术后短期复发转移的风险较高。
Objective To observe the preoperative expression of Yes-associated protein(YAP)in breast cancer tissues,and to investigate its correlation with short-term recurrence and metastasis after modified radical mastectomy in breast cancer patients.Methods Totally 213 patients underwent modified radical mastectomy.The serum tumor markers were detected including carbohydrate antigen(CA)125,CA15-3,β2-microglobulin(β2-MG)and carcinoembryonic antigen(CEA),and the expression of YAP was detected by immunohistochemical method in breast cancer tissues before operation.All patients were followed up for 1 year and divided into recurrence/metastasis group(n=50)and non-recurrence/metastasis group(n=163).The clinical data,the preoperative levels of CA125,CA15-3,β2-MG and CEA as well as the YAP expression in breast cancer tissues were compared between two groups.Multivariate logistic regression analysis was used to assess the influencing factors of short-term recurrence and metastasis after modified radical mastectomy in breast cancer patients.Results In 213 patients 1 year after operation,recurrence and metastasis occurred in 50 patients(23.47%).The preoperative levels of serum CA125,CA15-3,β2-MG and CEA as well as the negative expression rate of YAP in cancer tissues were higher in recurrence/metastasis group((37.38±4.26)u/mL,(36.84±5.21)u/mL,(2.61±0.27)mg/L,(1.89±0.15)μg/L,60.00%)than those in non-recurrence/metastasis group((34.72±4.81)u/mL,(34.32±5.17)u/mL,(2.43±0.24)mg/L,(1.80±0.14)μg/L,20.25%)(P<0.05).The preoperative levels of serum CA125(OR=1.137,95%CI:1.04-1.238,P=0.003),CA15-3(OR=1.104,95%CI:1.042-1.169,P=0.001),β2-MG(OR=16.408,95%CI:3.677-73.211,P<0.001),CEA(OR=18.695,95%CI:4.891-71.465,P<0.001)and YAP(OR=24.757,95%CI:1.758-348.738,P<0.001)were the influencing factors of short-term recurrence and metastasis after modified radical mastectomy in breast cancer patients.Conclusion Breast cancer patients with high preoperative levels of serum CA125,CA15-3,β2-MG and CEA as well as negative expression of YAP in cancer tissues have a high risk of short-term recurrence and metastasis after modified radical mastectomy.
